Rockwell International Letter 82ESG6297 dated August 20, 1982, to NRC Region IV, "Information Report on Noncompliance to ASME Code Test Requirement" Gentlemen:

Subject:

Information Report on Noncompliance to ASME Code Test Requirement The referenced letter reported an instance of a nuclear part that did not receive a pressure test to ASME Code Section III requirements prior to its acceptance. Evidence was arovided demonstrating that the use of this part, if the noncompliance lad remained undetected, would not have affected safety of plant operations. This letter describes the actions taken to correct this situation. Also, during preparation for testing, two additional administrative problems were found for this part which are described in the following paragraphs along with their corrective actions.

The part was returned from storage to Rockwell International Energy Systems Group in Canoga Park, California. It has been pressure-tested to design specification requirements under the cognizance of an Authorized Nuclear Inspector, and an ASME N-2 Data Report issued declaring the item an " appurtenance." The cause of this occurrence was failure to identify the housing fabrication specification as an affected document in the change control listing when the higher level design saecification was revised to reclassify the housing as a ") art" rather t1an an " appurtenance." The change control. procedure has >een revised to be significantly more expiteit on which type of documents are to be evaluated for impact when any document in the design hier-archy is revised.

October 24, 1983 International Page 2 Upon receipt of this item from storage, additional problems were found with the part and its Code certification. A review of the original N-2 Data Report disclosed that this report only covered the weld mate-rials and weld operations for the stainless steel overlay and plug welding. The basic forging was not covered by the Data Report. Also, the Data Report did not reference the post-weld heat treatment received by the housing. Finally, the ASME manufacturer's serial number on the part was an incomplete version of that on the N-2 data Report. A corrected N-2 Data Report was received from the supplier along with permission for ESG to complete the serial number on the housing. The serial number was applied under the cognizance of our Authorized Nuclear Inspector. The assembly containing the housing was returned to the CRBRP storage site on October 19, 1983.
