AMENDMENT TO FACILITY OPERATING LICENSE Amendment No.125 License No. NPF-6 1.

The Nuclear Regulatory Commission (the Commission) has found that:

A.

The application for amendment by Entergy Operations, Inc.

(the licensee) dated April 9, 1991, as revised August 30, 1991, complies with the standards and requirements of the Atomic Energy Act of 1954, as amended (the Act), and the Commission's rules and regulations set forth in 10 CFR Chapter I; B.

The facility will operate in conformity with the application, the provisions of the Act, and 'he rules and regulations of the Commission; C.

There is reasonable assurance:

(i) that the activities authorized by this amendment can be conducted without endangering the health and safety of the public, and (ii) that such activities will be conducted in complicnce with the Commission's regulations; D.

The issuance of this license amendment will not be inimical to the common defense and security or to the health and safety of the public; and E.

The issuance of this amendment is in accordance with 10 CFR Part 51 of the Commission's regulations and all applicable requirements have been satisfied.

Technical Specifications The Technical Specifications contained in Appendix A, as revised through Amendment No.125, are hereby incorporated in the license. The licensee shall operate the facility in accordance with the Technical Specifications.

The license amendment is effective as of its date of issuance.

mARrm The boron capability required below 200'T is based upon providing a 51 Ak/k SHVTDOWN MARGIN after manon decay and cooldown,from 200'F to 140'T.

This condition requires either borated water from the rafneling water tank or boric acid solution from the boric acid makeup tank (s) in accordance with the requirements of Specification 3.1.2.7.

The contained water volume limits includes allowance for water not available because of discharge line location and other physical characteristics.

The 61,370 gallen limit for the refueling water tank it based upon having an indicated legal in the tank of at least 7.5%.

The OPERABILITY of one boron injection systes during REWELING ensures that this systee is available for reactivity control while in MODE i

The limits on contained water volume and boron concentration of the RWT also ensure a pH value.of between 8.8 and 11.0 for the solution recirculated within containment after a LOCA. This pH band minimizes the evolution of iodine and minimites the effect of chloride and caustic stress corrosion on mechanical systess and coeponents.

3/4.1.3 MOYARLE CONTROL AEEEMPLTEE The specifications of this section ansure that (1) acceptable power distribution limits are maintained, (2) the minimus SNUTDOWN MARGIN is maintained, and (3) the potential effects of CEA misaliansents are limited to acceptable levels.

The ACTIDN statements which permit limited variations from the basic requirements are accompanied by additional restrictions which ensure that the original design criteria are met.

The ACTION etatements applicable to a stuck or untrippable CEA or a large misaligneont (219 inches) of two or more CEAs, require a prompt shutdown of the reactor since either of these conditione may be indicative of a possible loss of mechanical functional capability of the CEAs and in tbo event of a etack or untrippable CEA, the loss of gNUTDOWW MARGIN.

CEAs that are confirmed to be inoperable due to problems other than addressed by ACTION a of Specification 3.1.3.1 will not impact SKUTDOWN HARGIN as long as their relative positions satisfy the applicable alignment requirements.

For small misaligneants (( 19 inches) of the CIAs, there is 1) a small effect on the time dependent long tera power distributions relative to those used in generating LCOs and LSSS setpoints, 2) a small effect on the available SHUTDOWN MARGIN, and 3) a small effect on the ejected CEA worth used in the safety analysis.

REACTIVITY COh' TROL SYSTIwS RAuc statement associated with trippable but small misalignments of CEAs permits a one hour time interval during which attespts say be made to restore the CEA to within its alignment re,quirements. The one bout time limit is sufficient to (1) identify causes of a misaligned CIA, (2) take appropriate corrective action to realign the CEAs and (3) minimise the offects of renon redistribution.

Problems say also cause more than coe control rod to be immovable t ere the control rods continue to be trippable. With trippab)* but multiple inoperable rods; the alignment limits and the restriction on THERMAL POWER in accordance with the provisions of Specification 3.1.3.6 for insertirm limits, assures fuel rod integrity during continued operation. These provisions are sufficiant to allow 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> to restore the inoperable rods to operable status when it is confirsed that the cause of the immovable rods is an electrical problem in the rod control system or an electrical or mechanical probles with the rod stepping mechanism exclusive of the rod holding coil that must function for a reactor trip.

In such cases, the control rods will continue to be ctpsble of fulfilling their primary safety function.

The CPCs provide protection to the core in the event of a large misaligneont (? 19 imches) of a CEA by applying appropriate penalty factors to the calculation to account for the afsaligned CEA.

However, this misalignrent would cause distertion of the core power distribution.

This distribution say, in turn, have a significant offect on 1) the available SHUTDOVN MARGIN, 2) the time dependent long term power distributions reistive to those used in generating LCOs and LSSS setpoints, and 3) the ejected CEA worth used in the safety analysis.

Therefore, the ACTION statemant associated with the large misalignment of a CEA requires a proept realignment of the nisaligned CEA.

The ACTION statements applicabia to trippable but misaligned or l

inoperable CEA: include requireme.nts to align the OPERABLE CEAs in a given group with the inoperable CEA. Conforosoce with these aliansant requirements brings the ccN, within a short period of time, to a configuration consistent w'g b that assumed in generating LCO and L555 setpoints. However, antent.sd operation with CEAs significantly inserted it the core may lead to perhurbations An 1) local burnup, 2) peaking factoru.nd 3) available 5HUTDOVN MARGIN which are more adverse than the conditions assumed to arist in the safety analyses and LCO and L885 setpoints detereinstion. Therefore, time limits have been imposed on operation with inoperable CEAs,to preclude such adverse conditions from developing.

Operability of at least two CEA position indicator channels is required to determine CEA positions and thereby ansare compliance with the CEA alignment and insertion limits. The CEA " Full In" and "Fn11 Out" limits provide an additional independant means for determining the CEA positions when the CEAs are at either their fully inserted or fully withdrawn positions. Therefore, the ACTION statements applicable to inoperable CEA position indicators permit continued operations when the positions of CEAs with inop'erable position indicators can be verified by the " Full In" or " Full Out limits.
