Ooerating License No 2R-50 Docket No. 50-289 Overdue Licensed Operator Medical Examination On Friday, August ?,1991, while reviewing the licensed operator medical examination schedule, it was noted that one operator did not have a current medical examination. The operator in question was eturned to active status on July 18, 1991 following participation in a two-year college degree program.

10 CFR 55.21 requires a lictnsee to have a medical examination every two years.

The operator in question had not received a medical examination since Janutry 7, 1988.

Upon discovery of this problem, the operator was immediately removed from licuised duties. On Monday, August 5, 1991 the operator underwent a medical examination with no new medical restrictions identified. The operator subsequently was returned to licensed duties.

The review of the medical examination schedule overlooked this operator during the previous months while he had been participating in the college degree program and his license was restricted. All other licensed operators were verified to have current medical examinations.

To prevent future occurrences, two corrective actions have been taken. A Procedure Change Request h:.:; been initiated to add a step in procedure AP 1058,

" Operator Licenses" which will require a verification signoff for a current medical examination when a licensed operator is returned to active status.

It is expected that this procedure change will be implemented by October 1,1991.

Also a new operations surveillance OPS-5412 has been developed which will require a review of the medical examination schedule with a second review to assure its conpleteness.
