Dear Mr. Lewis:
This letter refers to the violation cited in the subject inspection reports which states:
"As a result of the inspection conducted on August 11 - September 10, 1983, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9,1982), the following violation was identified.
Technical Specification 3.3.3.8 requires that 'the accident moni-toring instrumentation channels shown in Table 3.3-11 shall be operabl e. '
Contrary to the above, on August 29,1983, at 2:00 p.m., the licensee discovered the Unit 1 containment sump wide range level inUcators (Item 15 on Table 3.3-11) de-energized and thus inoper-able. The indicators were inoperable for an unknown time, but probably since January 1983 - the previous refueling outage.
These wide range containment water level indicators have no auto-matic protection or control function. During this time frame, an increase in sump level would have been indicated by the two (2) operable narrow range containment level indicators."
Admission or Denial The above violation occurred as described in the subject report.

I Mr. R. C. Lewis November 10, 1983 Page Two Reason for Violation An intensive investigation into possible causes was conducted, with the following conclusions:
(1) Surveillance Test Procedures FHP-1-STP-206.1/FNP-2-STP-206.1 (Post Accident Containment Water Level Q1E11LT-3594A/Q2E11LT-3594A)and FNP-1-STP-206.2/FNP-2-STP-206.2 (Post Accident Contain-ment Water Level Q1E11LT-3594B/Q2E11LT-35948)did not specifically require the power switches to be left in the "0N" position follow-ing calibration, which had been last performed during the refuel-ing outage in February - March,1983.

Interviews with numerous I & C and Operations personnel produced conflicting recollections of whether the power-on lamps had been on or off during the 5-month period since refueling.
CorrecCve Action Taken and Results Achieved FNP-1-STP-206.1/FNP-2-STP-206.1 and FNP-1-STP-206.2/FNP-2-STP-206.2 were modified to require returning the power switches to "0N" following receiver calibration. Permanent Unit 1 and Unit 2 procedure revisions to include this change were issued by September 14, 1983. Operations daily logs have been revised to verify that power switches are on and power-on lights are illuminated.
Corrective Steps Taken to Avoid Further Violations See above. All corrective action was completed by September 14, 1983.
