Grand Gulf Nuclear Station Unit 1 Docket No. 50-416 License No. NPF-29 Special Report Commitment Revision to Special Report 90-003 GNR0-91/00151 Gentlemen:
On December 27, 1990, Entergy Operations reported a valid failure on the Division I Emergency Diesel Generator (SR 90-003).
Subsequent to that failure, another valid failure occurred on May 13, 1991 which was reported in SR 91-004. During root cause analysis for the subsequent failure, it was determined that the failure was a result of degraded lubricant between contact surfaces on the voltage regulator relay bridge selector switch.
In SR 90-003, Entergy Operations committed to inspect the carbon brush and the slip rings on a quarterly frequency and to periodically cycle the voltage regulator bridge relay selector switch. Based on the cause analysis of the subsequent failure, the committed actions in SR 90-003 will no longer be required.
Therefore, the commitment will be revise 6.
The carbon brushes and slip rings inspection will be returned to their original frequency.
