FIRST INTERIM REPORT OF SIGNIFICANT CONSTRUCTION DEFICIENCY No. 91

"GE AKR-30/AKR-50 CIRCUIT BREAKERS (SERVICE ADVISE 175-9.11)"

INTRODUCTION This report is submitted pursuant to 10CFR50.55(e). It describes a condition at Waterford SES Unit No. 3 relative to G.E. Safety-Related AKR-30 and AKR-50 circuit breakers which may fail to close upon command. (Service Advise 175-9.11).

This deficiency has been reported to the USNRC pursuant to 10CFR21, by General Electric Company, Atlanta, Georgia.

. DESCRIPTION The service advisory covered G.E. electrically operated AKR-30 and AKR-50 circuit breakers manufactured before May 1980 which may contain defective switch (es) and improperly heat treated part(s). This possibly could result in failure of these circuit breakers to close on command.

SAFETY IMPLICATIONS The G.E. AKR-30 and AKR-50 circuit breakers supply safety-related loads. If left uncorrected, and the circuit breakers failed to close on command, the safe operation and shutdown of the plant could be compromised.

CORRECTIVE ACTION TAKEN NCR-W3-7144 was initiated to replace all potentially defective switches and improperly heat treated parts installed on AKR-30 and AKR-50 circuit breakers.

A final or updated report will be submitted to the USNRC on or before January 24, 1984.
