Prior to the Bulletin being issued, the Babcock & Wilcox Company (B&W) sub-mitted a 10 CFR Part 21 Report to the NRC, dated January 10, 1983, regarding nonconforming material received from Tube-Line. B&W followed up this initial report with a letter to R C DeYoung, dated June 17, 1983, which outlined the investigations, actions and decisions that had been completed to resolve this problem. Consumers Power Company also identified the nonconforming material sent to the Midland Plant in their 50.55(e) report to J G Keppler dated February 28, 1983 and gave notice that this Part 21 issue was resolved in a 50.55(e) report dated May 31, 1983.

An NRC inspection of the Tube-Line facilities followed, the conclusion being that potential generic safety problems existed and that further investigations were required to assure that no T-L material was being used in safety-related systems, thus IE Bulletin 83-06 was initiated.

In accordance with the Bulletin, B&W is presently reviewing their QA data packages, which include the names of their suppliers, and comparing them with oc1183-0673a100 g

2 the lists of purchasing and receiving companies that were given in attachments 2 and 3 of the Bulletin. 1Following is a status of their review as of November 4, 1983.

I.. Control and Instrumentation (C&I) Related Equipment

-This category represents the safety-related pressure boundary components in the C&I Systems-A.

-There were approximately.34 QA' data packages to review for this equipment.

B.

All of the packages have been reviewed.

C..None of the T-L customers listed in attachments 2 and 3 of the Bulletin were identified as having supplied safety-related hardware to the Midland Plant.

II. ' Spare Parts for the Midlacd Plant This category represents safety-related equipment that has either been purchased or is on order.

A.

There were approximately 70 QA data packages to review for these parts.

B.

All of the packages have been reviewed.

C.

None of the T-L customers listed in attachments 2 and 3 of the Bulletin were identified as having supplied safety-related parts to the Midland. Plant.

. III. B&W Systems Other Than I and II Above A.

There were approximately 75 QA data packages to review for these j

systems.

Approximately 75% of these packages have been reviewed.

For-the percentage of work completed, no T-L customers have been L

identified as having supplied safety-related equipment to the Midland Plant.

A-review is also being conducted by the Bechtel Corporation to determine if their sub-suppliers have supplied Tube-Line material to the Midland Plant's safety-related systems.. Bechtel's approach to resolving this issue was to send letters to each of.their suppliers and have them review their records for

-instances where T-L material was ordered and/or used. Following is a status

- of their review as of-November 4, 1983.

67_ responses have been' received.by Bechtel'with only one supplier identified as providing T-L material, as described in C below.

C.

Guyon Alloys'have advised _that they have supplied T-L material to Midland. Five 90 degree SA-403 Ells are the only' items identified by material suppliers to date.

D.

Disposition.of these Ells is currently under investigation.

Letters Mailed.to Suppliers.0ther Than I Above:

A.

.There have been 303 letters mailed to these suppliers.

B.

189 replies have been received by Bechtel.

C.

0f the responses received, all advise that no T-L material was furnished to the Midland Plant.

The' letters Bechtel sent to their suppliers requested' replies within 30 days from receipt. : Follow up visits to suppliers were made by their Area Supplier Quality Supervisors and/or Supplier Quality Representatives to expedite replies from suppliers that had not' responded. A target date of Decent er 30, 1983, has been established for receipt of 100 percent of the replies.

.All Tube-Line material identified as being furnished to Hidland has beer entered on Nonconformance Reports (NCR) C-00355 and 6022. There were nite heat codes identified as being furnished to Midland through either B&W 01 Bechtel. -The four furnished to Babcock & Wilcox Company, EKP, EVVR, NDLG and NDUC, were either returned or recertified as indicated in the referenced 50.55(e) reports.

The five heat codes, as identified as being' furnished to Bechtel, have been processed as follows:

a) Heat Codes AABA, AABC, AACA and AAVF consist of 50 fittings, listed on NCR C-00355. Fifteen items have been located in the warehouse; the remaining 35 items are being researched by jobsite QA/QC to determine within what systems the items are installed. Target date of completion is scheduled for mid-December 1983.

Contacts have been made with Mr Vern Anderson, QA Director of Tube-Line.

Mr Anderson is to furnish additional documentation to substantiate the material /CMTR.

~b) Heat Code EWPB consists of two 10-inch caps furnished by Tube-Line to Capitol Pipe.to W J Wooley. The subject taps have been identified to ASME Class 2 air tank OT-921 on Purchase Order C-44, and listed on NCR 6022.

Capitol Pipe visited the Midland jobsite and has obtained scrapings from these. caps for chemical testing. Additional material from the same heat oc1183-0673a100

4 was obtained for physical testing. The additional testing results have been received by Bechtel Ann Arbor and forwarded to engineering for disposition.

These nine heat codes were confirmed as being sent to Midland when CPCo went to Tube-Line's facilities to perform an au6it. This audit was conducted as an added measure of assurance that all the T-L material at the Midland Plant would be identified and would also provide a check-list to compare results received from B&W, Bechtel and their suppliers. Tube-Line facilities were visited at Long Island City, NY, on August 15-17, 1983, and Carol Stream, IL, on August 30, 31 and September 1, 1983, to review the' reported problems, identify suspect Heat Codes, trace suspect materials to Big Rock, Palisades or Midland, and to track such remaining suspect materials to other installations.

This. evaluation was conducted by reviewing Sales Orders, Purchase Specifications,_ Interplant Orders, Packing Lists, Certified Material Test Reports, Certificate of Compliance, Certificate of Conformance, Invoices, etc.

The reported problems from IE Bulletin 83-06 were then traced to CPCo by Heat Code from the Tube-Line facilities.

The target date for the completion of identifying and dispositioning the suspect T-L material is tentatively scheduled for the end of' February 1984.

We expect to provide an additional response by March 16, 1984.
