ATTACHMENT I RESPONSE TO APPENDIX A VIOLATION A 10CFR50, Appendix B, Criterion IX requires that measures shall be established to assure that welding is accomplished by qualified personnel. The G.E.

General Welding Procedure LMI-GMP-1, Revision 0 specifies the requirements of ASME,Section IX for welder qualification as a project requirement.

ASME, B&PV Code,Section IX, Paragraph QW-303.2 states in part: " Welders who make fillet welds on pipe or tube less than 2 7/8" 0.D. must pass the pipe fillet weld test per QW-452.4.

Contrary to the above, on August 24, 1983, the inspector determined th'at two welders perf orming welds on in-core housing to guide tube socket fillet welds were not qualified to perform such pipe welds because they were qualified by plate fillet test assembly. The guide tubes are less than 2 7/8" 0.D.

RESPONSE TO VIOLATION A To determine the extent of the problem, a review of welders' performance records and the weld issue log was conducted. The performance records of two welders were found to be in tror and have been corrected. No additional instances of welders perrorming outside the limits of their qualifications were identified.

The welds made by tbc identified unqualified welders were removed and replaced by qualified welders.

To prevent a recurrence, the involved GE A&ESO subcontract personnel were instructed in the requirements of Section IX.
