SUMMARY

5 At the beginning of the month, the plant was in hot shutdown in the final stages of hot OTSG testing.

The primary system was at normal operating r

pressure, temperature and flows. The secondary system was also operating but at below capacity, f

The following is a summary of the months major events:

1 10/1 Commenced adding KR 85 to Make-Up System to detect primary-secondary OTSG leakage 10/2 Secured feed water and commenced steam down of both OTSGs; Started Emergency Feed Pump EF-P-2B and commenced RCS cooldown; Re-heated RCS to normal operating temperature / pressure / flow 10/6 Commenced RCS cooldown; Returned RCS to decay heat cooling e

10/7 T1CS returned to cold shutdown; Both 01SGs returned to full wet layup 10/29 Commenced RCS depressurization and partial drain down to l9 permit repair of Decay heat. valve DH-V1 10/31 Partial RCS drain down completed Major Safety Related Maintenance OTSC Repair Program The repair program that was commenced in December,1981 and has been covered in monthly reports since that time was completed this month.

Snubber Testing and Repair A program to test and repair as necessary selected plant snubbers

-continued through the month and into November.

i BS-V3A Repairs l-

' Repairs to Building Spray valve SB-V3A were required to correct leakage past the valve seat.. Post maintenance testing was performed satisfactorily.

DH-V1 Repair Decay Heat valve DH-V1 is a non-isolable 12 inch valve in the suction line that comes off the "B" loop hot leg.

It was discovered to have a bonnet seal ring leak approximately 5 years ago. A temporary repair then was -

made by injecting furmanite into injection nozzles drilled into the valve body.

During. October, maintenance activities to support disassembly and permanent repair of the valve included planning and fabrication of disassembly tools.

Actual disassembly was commenced on the last day of
