6-5-91 Scram during Group 1 Valve Isolation surveillance testing.

One half of a Group 1 isolation signal was 6-5-91 manually inserted for the testing when the other half of the isolation signal spuriously occurred for a very brief time.

6-5-91 to Generator off-line for scram recovery.

6-6-91 Plant startup after scram recovery.

High Safety Relief Valve tailpipe temperatures indicated a leaking SRV.

As power was being decreased for a plant shutdown to repair the SRV, a 6-6-91 reactor scraa occurred due to IRM Hi Hi signals caused by reactor cooldown resulting from insufficient decay heat in the core.

Decay heat was low due to only 4.5 days of operation following refueling.

6-6-91 to Generator off-line for SRV repairs.

6-8-91 6-8-91 to Generator on-line.

Power operation.

6-30-91 Note: Power operation defined as essentially 100% of rated power except for weekend load drops for specified surveillance testing.
