DOCKET NOS. 50-266 & 50-301 REPLY TO NOTICE OF VIOLATION INSPECTION REPORT NOS. 50-266/91008 AND 50-301/91008 POINT BEACH NUCLEAR-PLANT, UNITS 1 AND 2 By letter dated June 6, 1991, the subject inspection report and Notice.of-Violation were transmitted to us.

The violation cited three procedures as examples of documents for which the existing review measures were inadequate to prevent incorrect procedures fromDbeing issued.

We acknowledge the violation as stated.

Corrective actions taken to dateLinclude:-

1.-

Review and discussion of this violation by the Manager's Supervisory Staff-on June 18, 1991.

This discussion is documented in the meeting' minutes.

2.

Establishment of an.ad hoc committee-to investigate the details of each example of this violation to determino the root cause for the events.

The committee met on June 26, l'

1991.

The root cause of the events was determined to be L

~

essentially as stated in the inspection report.

The committee determined that PBNP 2.1.1,

" Classification, Review, and Approval of Procedures," and the forms associated with the review process are inadequate in that the term

" review" is not defined.

In addition, the committee recognized that accountability for

. achieving and maintaining high standards of quality needs to be emphasized to all persons involved in the life cycle of a procedure, from the original author.to the end user.

The results of the committee's review and its recommendations were presented to a quorum of the Manager's Supervisory Staff during a meeting held on June 27, 1991.

The recommendations were:

a.

Perform a detan ed evaluation of PBNP 2.1.1 and its associated forms, with special emphasis being placed upon defining the level and detail of reviews which are conducted for a procedure.

This activity will be completed by October 4, 1991.

b.

Develop and implement measures which emphasizes "self checking" during the life cycle of a procedure.

It is anticipated that these actions will be completed by January 31, 1992.
