l ENCLOSURE COMMONWEALTH EDISON COMPANY RESPONSE TO INSPECTION REPORT NOS: 456/91013;457/91011 VIOLATION:
Technical Specification 6.8 requires, in part, that written procedures for plant operations as recommended in Appendlx A of Regulatory Guide 1.33, Revision 2, February 1978, shall as established, implemented, and maintained.
Contrary to the above, on April 17,1991, the licensee did not correctly implement a written procedure required by Regulatory Guide 1.33, by failing to open the mixed-bed domineralizer outlet valve per step F.1.d of BwOP CV 8, "CV System Mixed Bed / Cation Demineralizer Operation."
RESPONSE
Commonwealth Edison Company (Edison) acknowledges that procedure BwOP CV-8, "CV System Mixed Bed / Cation Domineralizer Operation," was incorrectly implemented during the switching of the Unit 2 mixed-bed demineralizers in the Chemical and Volume Control System. The individual performing the evolution suspected air entrainment in one of the domineralizers and was attempting to vent the demineralizer prior to placing it in service. The individual performed steps that were not included in the procedure in order to ensure that the demineralizer was vented. During this process a pressure spike occurred that may have contributed to the failure of the diaphragm in the 2A chemical and volume control mixed-bed demineralizer inlet valve (2CV8524 A) allowing a small spill to occur contaminating one individual and the immediate area. However, it has been determined that the valve was designed to withstand the pressure experienced. The valve diaphragm appears to have failed premature'y.
Edison's review of this event determined there has been no adverse impact en the health and safety of the public.

'CORREC flVE ACTION TAKEN AND RESULTS ACHIEVED:
The contaminated Individual and the contaminated area were successfully decontamlaated using standard decontamination techniques.
The damaged valve was repaired under the Nuclear Work Request Pregram.
The indiv! dual involved in this event was counseled on the importance of following procedures.
CORRECTIVE ACTION TO PREVENT FURTHER VIOLATION:
The Operating Department's tallgate training session which started on May 28, 1991 stresses the importance of procedural adherence.
Appropriate operating shift personnel will receive training on this event during licensed operator requalification training. This training will emphasize the importance of procedural adherence and the actions to be taken when the procedures do not fully address the operation to be accomplished.
Braidwood Station Procedure BwOP CV-8, "CV System Mixed Bed / Cation Demineralizer Operation," will be revised to include directions on venting the domineralizers when the system is in its normal operating mode.
DATE OF FULL COMPLIANCE:
Braidwood Station Procedure BwOP CV-8, "CV System Mixed Bed / Cation Demineralizer Operation,"is expected to be revised by July 1,1991.
The tallgate training sessions are expected to be completed by July 8,1991.
The licensed operator requalification tralning is expected to be completed by October 31,1991.
