Description of Violation (50-334/94-27-01)
Unit i Technical Specification 6.8.1 requires that written procedures shall be established, implemented, and maintained covering refueling operations. Definition B.5 in Beaver Valley Power Station Refueling Procedure IRP-10R-1.1 states that a QC Hold Point requires QC verification of scope of work completion prior to commencement of.
subsequent work.
Contrary to the above, on January 13, 1995, the licensee removed the Unit 1 reactor vessel head without completing the head lift rig critical weld inspection specified in the QC Hold Point of Step 14 of Beaver Valley Refueling Procedure 1RP-10R-2.7.
Specifically, the QC Hold Point in Step 14 of Beaver Valley Refueling Procedure IRP-10R-2.7 states "The reactor vessel head has been raised approximately 2 inches, checked level and held for 10 minutes. Visually inspect the head lift rig critical welds and parts as listed in Tables 1 and 2. If no problems are apparent, continue to lift, monitoring the load cell readout at all times. No cracks or deformation shall be apparent." Only ten of the twenty-five points listed in Tables 1 and 2 of IRP-10R-2.7 were inspectect prior to lifting the reactor vessel head past the 2 inch point specified in the procedure.
Reason For The Violation J
The reason for the violation was personnel error. The Refueling Assistant did not ensure that the procedure was performed as specified and did not verify that the QC Inspector completed the required inspection before lifting the head beyond the 2 inch point j
specified in the procedure.
Corrective Actions Taken immediately after the completion of the reactor vessel head lift, performance standards were reviewed with the Refueling Supervisor. It was determined that these standards were being met with the exception of the event described in the violation.
The Refueling Assistant and the QC Inspector were removed from refueling activities and disciplined accordingly.

Reply:to Notice'of Violation Cdmbined Inspection Report 50-334/94-27 and 50-412/94-28 Page 2 Corrective Actions Taken (Continuedh Assigned refueling personnel were retrained on performance standards. QC inspection personnel were retrained on QC Inspector independence and the requirements for signing QC Holdpoints. Field monitoring was performed to ensure that training was effective.
A detailed case study and root cause evaluation of the event was completed.
The lift rig inspection was completed during reactor vessel head re-installation.
Actions Taken To Prevent Recurrence Field monitoring of refueling and quality control performance standards will continue to ensure standards are maintained.
