ATTACHMENT A FINDING:

10 CFR 50, Appendix B, Criterion X, and the GPU Nuclear Corporation Opera-tional Quality Assurance Plan, Section 6.2.1.1, require inspections of

. activities affecting quality to verify conformance with documented procedures and drawings.

Contrary to the above, as of July 8, 1983 2dequate inspection of activities affecting quality to verify conformance with documented procedures and drawings was not conducted in that an anchor bolt associated with modifi-cation task RM-133, Engineering Change Memorandum 198, Main Steam Rupture Restraint to Protect EFW Pipe, did not have full thread engagement of the nut as required by Mechanical Corrective Procedure 1410-Y-61, Revision 0; and a weld associated with the same modification was of a smaller size than that specified by drawing ID-411-24-001. The nonconforming conditions were accepted by Quality Control inspection as documented on Anchor Installation Document RM-13H-B and Plant Inspection Report Number CS/30776/81.

This is a Severity Level V violation (Supplement I).

RESPONSE

I. Corrective ~ Steps Taken and Results Achieved During the NRC inspection in which the violation was identified, Quality Control on July 7, 1983 issued MNCR 0194-83 in order to initiate corrective action for the two conditions noted in the violation. These specific conditions were subsequently evaluated by Technical Functions and found to be acceptable in the as-found configuration. It was noted that the conditions were not in full conformance with the applicable requirements.

II. Corrective Steps Which Have Been Taken to Avoid Further Violations The cognizant' production and quality control personnel have been reinstructed in the requirements for proper installation and inspection of these supports

- in order' to prevent recurrence of the violation.
