Calven Cliffs Nuclear Power Plant Unit Nos.1 & 2; Docket Nos. 50-317 & 50-318 Annual Report of Failures and Challenges to Power-Operated Relief Valves and Safety Valves for the Period of Januarv 1.1994 through December 31.1994 This report is submitted in accordance with Technical Specification 6.9.1.5.c. The Technical Specification requires an annual report be submitted to the Nuclear Regulatory Commission documenting all challenges to, and failures of, the pressurizer Power-Operated Relief Valves (PORV) and Safety Valves.

On July 19,1994, Unit 1 tripped from 100 percent power due to a Reactor Protective System actuation after all four main turbine stop valves unexpectedly closed. During the resulting transient both pressurizer PORVs, ERV-402 and ERV-404, opened as designed. Safety valve RV-201 lifted at a pressure significantly below its set pressure and then leaked at an estimated rate of 25 gallons per minute. It was determined that the disc holder to RV-201 was improperly staked to the bellows assembly causing the valve to leak and then prematurely lift when it was exposed to the conditions generated by ERV-404 lifting in the same relief train. The event is described in further detail in Licensee Event Report 50-317/94-007.

There were no challenges or failures of the Unit 2 PORVs or Safety Valves during 1994.
