ZIMMER NUCLEAR POWER STATION - UNIT 1 10CFR50. 55 (e) ITEM E-49 ESSENTIAL 480V SUBSTATION COOLING FANS W.O. 57300, JOB E-5590, FILE NO. 956C, E-49 This letter constitutes a final report concerning the subject condition initially reported to the Commission on August 23, 1983 under the requirements of 10CFR50.55 (e). This condition is a reportable deficiency representing a significant deviation from performance specifications which could adversely affect the safe operation of the plant. Zimmer Project has been reviewing prior Nonconformance Reports in the Quality Confirmation Program and other quality reviews. These reviews occasionally identify conditions which occurred years ago and which may have represented significant deficiencies under the requirements of 10CFR50.55 (e). In response to 10CFR50. 55 (e), Item E-44, CG&E committed to identify such significant deficiencies found in these reviews, even though correction of the deficiency was completed in past years. l l The condition described in this report was found in review of i Nonconformance 80-5-E of 1/31/80. It relates to I.T.E. Imperial Corporation essential 480V substations containing fan cooled, 750/1000kVA AA/FA, dry type powe r transformers. The holding nut for the cooling fan blade worked loose which resulted in elongating the metal centering washer. Continued operation of the fan could have led to the blade being thrown into the transformer windings. In addition, the loss of the cooling action from the fans during full-load operation could have i resulted in damage to the windings by overheating. h 8309290044 030921 6 ej 9. 2 PDR ADOCK 05000358 -{f, g g S PDR \\Q 5

Keppler Regional Administrator September 21, 1983 LOZ-83-0153 Page 2 The corrective action which has been taken to address this condition, as recommended by the manuf acturer, was to replace the nuts on the f an blades with locking washers and nuts. This work has been completed for the essential substations and the appropriate plant documents are being changed to provide correct guidance for future maintenance of these fans. CG&E considers this deficiency to be a vendor related design deficiency and has notified the vendor for 10CFR Part 21 evaluation.
