57300, JOB'E-5590, FILE NO. 956C,'E-51 This letter constitutes an interim report concerning the subject condition initially reported to the Commission on August 24, 1983 under the requirements of 10CFR50.55(e).
This condition represents a reportable deficiency in final design that could adversely affect the safe operation of the plant.
Zimmer Project has been reviewing prior Nonconformance Reports in the Quality Confirmation Program and other quality reviews.
These reviews occasionally identify conditions which occurred years ago and which may have represented significant deficiencies that were reportable under the requirements of 10CFR50.55(e).
In response to 10CFR50.55(e) Item E-44, CG&E committed to identify such significant deficiencies found in these reviews, even though correction of the deficiency was completed in past years.
The condition described in this report was found in review of H.J. Kaiser Nonconformance Report E-538.
It was identified that there was no approved design detail or installation specification requirement to ensure separation barriers in control room panels were installed in compliance with the purchase specification.
The specification requires all barriers to completely isolate (floor to ceiling and front to rear) the essential zones within the control board.
The lack of design detail or installation specification requirements has resulted in two types of conditions.
The first conditon resulted when the panel was lifted and placed on the channel base leaving a 3 - 4 inch gap between sections.

The second condition was caused by the alignment between the panel door and the separation barrier which allowed a 1 inch gap to exist around the barrier between redundant panel sections.
This condition was identified only on panels IPM08, IPM07J, and 1PM01J.
Both conditions were initially identified on Nonconformance Report E-538.
A Field evaluation of these conditions indicated that the gap in the separation barrier would have resulted in a violation of the criteria jeopardizing redundant divisions and could adversely affect the safe operation of the plant.
The corrective actions which will be taken to resolve the hardware deficiencies are as follows:
1).
A 1 inch thick marinite XL barrier will be installed to close the space at the bottom of the barrier for each panel.
2).
A firc blanket or a 1" marinite XL panel will be attached to the panels where barriers do not align with door ends.
The design changes detailing the revisions have been incorporated onto design documents.
A date when a final report will be submitted cannot be projected at this time.
Completion of the corrective actions are contingent upon the release or relaxation of the NRC November 12, 1982 " Order to Show Cause and Order Immediately Suspending Construction" for work in this area.
A projected schedule for completion of the above corrective actions will be submitted at that time.
CG&E is continuing to evaluate the adequacy of erection / installation criteria in separate 10CFR50.55(e) Item M-87 to determine if further correctiva action is required to prevent future similar occurrences.
