The following is a clarification of our letter of December 14, 1983, which responded to Inspection Report 50-366/83-29:
Our response under Corrective steps which will be taken to avoid future violations implied that Plant Review Board (PRB) members were solely responsible for review of revisions to procedures.
This is not the case.
Prior to PRB review, the supervisor of the person revising the procedure and the appropriate department manager review and must approve the procedure revision.
The department manager documents his review on the procedure revision request form.
The procedure revision is then transmitted to the PRB reading) room where each PRB member individually reviews and comments (as appropriate on the revision. The revision, along with all comments is then discussed during the IRB meeting, at which time it is rejected or approved.
The PRB is responsible for assuring the accuracy of procedure revisions, but other personnel are involved in the revision process.
The Corrective steps which will be taken to avoid future violations, as delineated in the hecember 14, 1983, letter, should have included the following:
The individuct responsible for making the procedure revision discovered the error in the procedure.
He was made aware of his responsibility to ensure the technical accuracy of procedure revisions prior to P..B submittal.
As a result of LER 50-366/83-39 and the reference inspection report, both the PRB and the department managers were made aware of the need for careful review of procedure chang s, particularly those affecting safety-related systems.
Emphasis was placed on the departmental manager's responsibility to assure accuracy prior to PRB review.
