Report of 10CFR50.55(e) Item on Broken Valve Stem on 24" Globe Valve and Pilot Pin (#109)
Dear Mr. Keppler:
On January 3, 1984, Detroit Edison's Mr. J. E. Durkin, Quality Engineer - Construction Quality Assurance telephoned Mr. J. E. Konklin of NRC Region III to report a broken valve stem on a 24" globe valve and pilot pin at the Fermi 2 site. The pilot pin on the lower portion of a disc of the 24" globe valve broke off the disc. This allowed the disc to vibrate putting stress on the valve stem causing the stem to break. This appears to be a fatigue f ailura, and is currently being evaluated by Detroit Edison's Engineering Research. Detroit' Edison has ordered new parts for the 24" globe valve from the vendor. Detroit Edison's Engineering Research has not determined the mode of failure. The research and evaluation will continue until the cause is determined. Another report on this item, either interim or final, will be sent when further information is available. If you have questions concerning this matter, please contact Mr. G. M.
