Dear Mr. Gagliardo:

This letter is written in response to your letter of December 12, 1983, which transmitted Inspection Report STN 50-482/83-34. As requested, the Violation identified in the Inspection Report is being addressed in three parts:

a) The corrective steps which have been taken and the results achieved; b) Corrective steps which will be taken to avoid further violations; and c) The date when full compliance will be achieved.

VIOLATION: Failure to Follow Construction Procedures Finding:

10CFR50, Appendix B, Criterion V, requires, in part, that activities affecting quality shall be accomplished in accordance with pre-scribed instructions, procedures or drawings.

Daniel International Corporation Procedures WP-X-303 and QCP-X-303, both titled " Instal-lation of Cable" and the cable installation work authorization cards require certain power cables to be spaced by one diameter in the supporting cable tray in order to prevent cable overheating during operation of the components powered by such cables.

and penetration ZNI268, located in the North Penetration Room, are not secured such that the one full diameter cable spacing is achieved. According to documentation on file, this instal-lation has had a final quality control acceptance.

Responses a) Corrective steps which have been taken and results achieved:

Nonconformance Report (NCR) ISN14124E was initiated to resolve the identified discrepancy. The cables have-been reworked and the NCR has been closed.

b) Corrective steps which will be taken to avoid further violations:

Procedure WP-X-303 requires one full' diameter spacing between cables of the type in question unless specified o therwise. For the particular power cables in question, Drawing E-OR1441 specifies 1/4. diameter (5/8") spacing.

Subsequent to quality acceptance of the cable in question, per procedure QCP-X-303 " Inspection of Cable Installation,"

the specified 1/4 diameter minimum cable separation was violated.

During normal construction activities, cable configuration can and does occasionally become altered from the original accepted condition. Therefore, once the raceway is complete there is an additional acceptance inspection performed.

This inspection is required by procedure QCP-X-300 " Inspection of Electrical Raceways" and specifically verifies cable bedding. Therefore, procedures are in place to assure proper cable separation once the-raceway is complete. The-raceway containing the cables in question had not yet under--

gone the vinyl installation inspection in accordance with QCP-X-300.-

c) Date when full compliance will be achieved:
