GUIDANCE FOR IDENTIFIED ISSUE - ANO 2 (DN 50-368) (AITS F40235H2)

This responds to your memorandum to X. V. Seyfrit, dated 7/18/77, request-ing guidance on acceptable locking devices and control of these devices for manually and power (manual switch and automatic) operated components.

The position IE inspectors should take regarding locking devices and their control for manually-o;:erated valves is as follows:

1.

A tag or similar device on a valve handwheel does not meet the requirements for a locked valve in a fluid system important safety.

2.

To be locked, according to Webster's New World Dictionary, is to be " fastened by means of a lock". A lock is defined in the same reference as "(1) a mechanical device furnished with a bolt and usually a spring, for fastening a door, strongbox,'etc., by means of a key or combination", and "(2) anything that fastens something else and prevents it from opening, turning, etc." These definitions should be sufficient to preclude the use of a single tag on a handwheel as a lock, but would permit a variety of mechanical devices to terve the purpose of a lock if they prevent opening, turning, etc., and require a key or combination for unlocking.

I Regarding locking a oower-coerated component into or out of an operating mode, the position to be taken is as follows:

Electric power should be removed by positive action, such as removing fuses or racking-out circuit breakers in circuits supplying power to the components. Administrative procedures l.

G. L. Madsen 2-should include additional provisions such as tagging the fuse holder or box or the breaker, and specific instructions as to what steps must be taken to restore power to the components involved.

The above. positions are applicable to items specifically designated in the application or TS as being locked.

Manual or power operated components for which requirements are not specified in the Technical Specifications or the application may be controlled by whatever procedures the licensee has developed for that purpose.
