Dear Mr. Seidle:

This letter is written in response to your letter of June 14, 1983, which transmitted Inspection Report STN 50-482/83-13. As requested, the violation identified in the Inspection Report is being addressed in three parts:

a) The corrective steps which have been taken and the results achieved; b) Corrective steps which will be taken to avoid further violations; and c) The date when full compliance will be achieved.

Violation Finding: Failure to Follow and/or Implement Quality Requirements 10CFR Part 50, Appendix B, Criterion V, requires, in part, that activities affecting quality shall be prescribed by instructions, procedures, or drawings and shall be accomplished in accordance with these instructions, procedures, or drawings.

Daniel International Corporation (DIC) Construction Procedure AP-VI-01, Revision ll, " Indoctrination, Training, and Certification of Quality Personnel," paragraph 4.5.1 states in part:

"All Quality Inspection, Testing, and Examination Personnel shall have an eye examination for near vision and color vision." This pro-cedure requires that the eye examination be in accordance with ANSI N45.2.6-73 (Qualification of Inspection, Examination, and 83083OO754 [ h 2 PDR ADOC PDR O

Testing Personnel for the Construction Phase of Nuclear Power Plants).

e.NSI N45.2.6-1973, paragraph 3.2.1 states in part: "Each person who perform inspections, examinations, and tests shall be physically capable of performing his assigned task. Characteristics that should be considered are:

(1) Natural or corrected near-distance acuity such that the applicant is capable of reading J-1 letters on standard Jaeger's test type chart for near vision or equivalent test type.

DIC uses the Bausch and Lomb Ortho-Rater test and line 10 or higher is equivalent to the J-l letters on the standard Jaeger's test.

Contrary to the above, 36 quality control personnel who perform inspections, examinations, and tests were not capable of reading J-l letters on the Jaeger test.

Records indicated that they could only read line 9 or below on the Ortho-Rater examination.

Response

a) Corrective steps which have been taken and results achieved:

The appropriate Quality Training personnel have been instructed in the correct method of evaluating and interpreting eye examination results and the correct method has now been implemented. A review of personnel certification files has been performed to determine which Quality Inspection personnel (past and present) were awarded certification based upon erroneous eye examination grading methods.

b) Corrective steps which will be taken to avoid further violations:

Those on-Site personnel who did not meet the minimum (Jaeger 1/

Ortho Rater 10) acuity level will be reexamined to confirm the original results. Should any of these individuals fail to meet the required level of near distance acuity, he shall be removed from inspection activities until such time that vicion is corrected' to acceptable limits, or independent analysis can determine that Jaeger 1 capabilities are/have been met.

For those individuals who meet the required level upon reexamination and evaluation, no further action is required. For those individuals no longer on Site, or on-Site personnel who have

c) Date when full compliance will be achieved:

Full compliance will be achieved by September 1, 1983.
