AFFIDAVIT OF RONALD G. TCLSON REGARDING "OPEN ISSUES" IDENTIFIED IN PROPOSED INITIAL DECISION I,

Tolson, being first duly sworn do depose and state as follows:

I am employed as the Site Quality Assurance Supervisor at Comanche Peak.

As such, I am familiar with the QA Program at Comanche Peak, including the QC inspection processes.

I have previously testified in this proceeding regarding the QA Program at Comanche Peak.

A statement of my educational and profossional qualifications was received into evidence as Applicants' Exhibit 20.

This af fidavit addresses two "open issues" identified by the Board in its July 29, 1983, Proposed Initial Decision.

The "open issues" which I address concern the Hilti Bolt verification process and an allegation regarding nondestructive examination on the fuel pool liner.

Hilti Bolt Verification Process The Board has identified as an "open issue" the adequacy of

the Hilti Bolt verification process.

would disclose instances in which Torque Seal had been applied to Hilti Bolts by craft personnel, rather than by or in the presence of OC inspectors.

As I discuss below, the record of Hilti Bolt inspection involves verification of several attributes other than the application of Torque Seal.

The procedures applicable to Hilti Bolt inspection provide that QC inspectors either witness directly the torquing operation or verify that the proper torque value was achieved following torquing by craft personnel.

Specifically, the following information is to be documented on the inspection report:

(1)

Verification that the torque wrench is cali-brated and the calibration is current.

(2)

Verification that the torque wrench setting is in accordance with the applicable re-quirements for the bolt being torqued.

(3)

Verification that the correct size socket for the bolt being torqued is used.

(4)

Both the M&TE number ( torque wrench number) and calibration due date.

The inspection report for each support or fixture which is secured by Hilti Bolts will also include a unique identification number for that support or fixture.

Torque Seal is applied to the bolt following the torquing operation.

See QI-QP-ll.2-3,

e 4 In sum, whether a QC inspector personally applied or observed application of the Torque Seal on the bolt is not alone significant.

The significant consideration is that there must be a complete inspection report for each support or fixture or a reinspection and reverification of torquing will be required.

Fuel Pool Liner Another "open question" identified by the Board concerns an allegation that a craf t person was performing liquid penetrant testing on the fuel pool liner.

Apparently, the Board had drawn the implication from the testimony in this proceeding that that person was performing the testing for QC purposes.

Proposed Initial Decision at 39.

However, as I discuss below, an investigation of this allegation revealed that this testing was not being performed for QC purposes.

The NRC conducted an investigation into this particular allegation.

During that investigation it was determined that the craft person who was observed performing liggfdpenetranttesting was performing in-process examination of work for craft purposes prior tf1 formal QC inspection and testing.

Craft personnel may perform such testing for their own purposes, although that test-ing is not recognized by QC for inspection and testing purposes.

used when verifying or witnessing torquing and spacing of concrete anchor bolts.

Bolts which cannot be torqued to the i

minimum values given in Table 1 below will be cut off and driven into the hole. The hole shall be patched as per Reference 1-A.

3.0 INSTRUCTION 3.1 SCHEDULE OF REQUIRED TORQUING The witnessing of torquing of concrete anchor bolts shall be accomplished as per Reference 1-B.

3.2 ANCHOR BOLT TORQUE REQUIREMENTS I

Anchor bolts shall be torqued as per Table 1 below:

M 2 5 P382 2 of 10 3.3 ALTERNATE METH005 0F TENSIONING When torquing cannot be accomplished, or is not desi rable, concrete anchor bolts may be tensioned by the turn-of-the-nut method. Turn-of-the-nut is accomplished by rotating the nut from a hand tight condition some incremental amount until the required tension in the bolt is achieved.

If this method is to be used.

Engineering personnel shall provide the values. The QC Inspector will witness the initial marking and rotation as per Reference 1

1-B in lieu of torque requirements.

3.4 TORQUE WRENCH Prior to craftsmen perfonning torquing, the following attributes shall be verified by the QC Inspector:

a.

Verify the torque wrench is calibrated and the calibration

This is accomplished by reviewing the cali-i bration due date on the calibration sticker on the torque wrench.

b.

Verify the setting of torque set on the wrench is as per 1

Section 3.2 of this Instruction.

Verify the correct sized socket for the bolt being torqued is used.

Record the M&TE number and calibration due date on the IR (Attachment 2).

3.5 HILTI BOLT The QC Inspector can either witness the torquing operation, or alternately verify the torque value after t'le fact.

The decision of whether to witness or verify the torquing operatico shall be made by the QC Inspector based on the mount of Hilti l

bolts installed by the craft per shift.

l The engagement of the nut shall be such, that after torquing, the end of the bolt is not lower than flush with the top of the l

nut.

The projection of. the bolt beyond outer surface of the nut is not limited, although a change in its j tion during one (1)g (when witnessed by the QC inspector) pro ec i

3.6 RICl#40ND INSERTS Verify that engagement into the Richmond Insert equals or exceeds 2 times the diameter of the bolt or threaded rod plus 1/8 inch. Also verify that the bolt is " snug tight."

NOTE:

Engagement into Richmond Inserts may be verified by Ultrasonic Testing or physical measurement of the bolt or all-thread before and after installation.

3.7 SEALANT If the Hilti bolt or Richmond Insert is installed and torqued / tightened accordingly, apply an appropriate amount of Inspector's Lacquer to torque seal the bolt.

3.8 HILTI BOLT SPACING When verifying or witnessing Hilti bolt torquing per the torquing requirements of Reference 1-8, concrete anchor bolt spacing for all the bolts on that item shall also be checked.

Bolt spacing shall be as per Attachment 1 (Tables 1, 2, or 3).

When measuring distance between bolts, it shall be measured from center to center of the bolts.

NOTE:

Concrete anchor bolts detailed on an individual fixture drawing need not follow the spacing requirements, as these have been approved in accordance with the design procedures.

Spacing violations between fixtures that are acceptable to Engineering will be documented on Attachment 6 of Reference 1-C by the affected engineer (s).

This shall be considered proof positive that an evaluation has been performed and is acceptable.

3.9 HILTI BOLT EMBEDMENT When verifying or witnessing hilti bolt torquing and spacing.

This shall be accomplished by subtracting the length of bolt extending from the structural concrete surface (including baseplate thickness and topping thickness if present) from overall bolt length.

Bolt length shall be determined from Table 4, Attachment 3 utilizing letter designation stamped on the bolt and the bolt diameter.

Minimum bolt embedment shall be as specified on Table 5 of Attachment 3 or the applicable drawing.

If no letter designation exists on the bolt, the inspector shall denote this condition as unsatisfactory on the IR.

Bolt length may then be detemined by Ultrasonic Testing.

If embedment is found acceptable, the IR shall be closed and the UT Report shall be referenced on the IR.

3.9 DOCUMENTATION Verification and witness results shall be documented on an Inspection Report (Attachment 2).

The item / fixture shall be identified by its unique number, if any, or else by its location.

The IR shall be processed as per Reference 1-C.
