Dear Mr. Ilughes:

This letter is written in response to the Notice of Violations submitted to River Bend Station (RBS) by your office dated October 26,1994. These violations were incurred during the Class I survey conducted on October 24,1994 at RBS. The following .

responses document the corrective action taken by RBS management to correct each l violation as stated in the original survey.

4 1 Violation #1 - Low spots observed around well Pl A will be corrected by grading the area  ;

surrounding the well so that storm water is diverted away from the well. j Violation #2 - Two station work requests will be worked to restore the physical condition of wells P1 A and P1B by removing corrosion, cleaning the base metal, priming and repainting to prevent future deteoriation of the well casings. j Violation #3 - A printed copy of Chapter 12: 003-1 of the La. St. San. Code was posted in

.i the following areas of the plant: Environmental Services, Chemistry, Maintenance, Buildings & Grounds, System Engineering and Operations departments.

Violation #4 - Temporary Certification Applications were submitted for the following individuals to operate the River Bend drinking water system as per requirements of the State Operator Certification Act:

Violation #5 - The air relief valves on drinking water wells Pl A and PIB will be reoriented to the downward position and screens will be installed in the ends of the pipe to prevent intrusion of contamination. In addition maintenance will be performed on well PI A to correct the leaking pump condition noted during the inspection.

Corrective action for violation No.'s 1,2,3,4 and 5 referenced above will be complete by December 26,1994 as specified in the Notice of Violations submitted to us on October 26,1994. In addition to the three presently certified operators, six additional RBS management personnel wih be certined by the State of Louisiana. We understand that immediate corrective action for violation No. 4 will be satisfied by submitting applications for designated River Bend personnel to be temporarily certified and that we have two years from the date ofissue of temporary certifications to qualify applicants for a mandatory certificate.
