10CFR Part 21 Defect in Piping Subassemblies Supplied to Clinton Power Station

Dear Mr. Keppler:

In Illinois Power letter to Mr. Frank Jablonski, NRC Region III (Ref: U-10024, 1606-L, dated January 17, 1983), Illinois Power agreed to notify Region III of those 10CFR Part 21 defects that affect Clinton Power Station that were previously reported to the.Cammission by vendors or contractors. In accordance with that agreement, the following information is submitted:

On June 24, 1983, Mr. N. J. Moerke, Vice President -

Engineering, Southwest Fabricating & Welding Co. , Inc. , informed the NRC, Region IV, of an indeterminate quality of welds in four (4) piping spools supplied to Clinton Power Station. The report to the Commission stated that Southwest Fabricating apparently failed to comply with the ASME Code,Section III, Paragraph NC-4424, that requires the weld surface. irregularities to be removed prior to performing radiography of the welds. As a result, weld defects may be masked by weld surface irregular-ities.

Illinois Power Company is investigating this issue as part of a potential 10CFR50.55(e) deficiency (55-83-07) reported to the NRC, Region III on June 14, 1983 and will ensure that corrective action will be taken to resolve the problem.

During the telephone conversation with Mr. I. Barnes, Region IV RSCPS cn June 22, 1983 at 10:15 AM, I presented the initial notification of the SF&?CO apparent failure to comply with the requiremnts of ASFE III Paragraph 10-4424, prior to radiography. This apparent failure was reported to me on June 20, 1983 by Mr. R. P. Bornes, SF&WCD bbnager of Quality Assurance.

'Ihe branch connections, as described below, were 45 laterals with reinforcing pads. Radiography was performd prior to installing the reinforcing pads. Our stated position relative to this deviation is:

Weld quality is currently indeterminate due to surfacx2 irregularities that Iray mask defects.

During the discussion, Mr. I. Barnes had specific questions regarding the RT technique that was employed by Southwest on these items and recorded on the Reader Sheets prior to shipm2nt of the items, and also viewd at the Clinton, IL jobsite. Therefore, Mr. Bornes, QA bbnager and ASB.'r I22 vel III, who viewed the radiographic film and verified the radiographic film deficiencies at the jcbsite on June 16 and 17, 1983 recorted his observations in detail to Mr. I. Barnes. Our Mr. Bornes discussed the individual characteristics of the film with Mr. I. Barnes.

l Nuclear Regulatory Comnission June 24, 1983 Page Two It was pointed out that the radiographic filns undergoing the review at this late date, were accepted by the Baldwin Associates QA repro-sentative and the Hartford Authorized Nuclear Inspector, wtlo signed the Manufacturers Data Peport after the subject radiographic film was released by Southwest, prior to code starping the items NPT C1. 2.

It should be recognized that radiographs of branch connections taken before the reinforcement pad was installed were evaluated and accepted by our personnel and the ANI with many surface irregularities shown on film. At that tine, the weld was accessible for visual examination, the joint configuration and the welding processes were available, and this information was used in the evaluation process. Obviously, all of this information, including the weld surfaces, is no longer access-ible and makes it inpossible to assure that indications shown on the film arc- surface inperfections.

In accordance with thd 'requirenents of Paragraph 21.21(3), we are tabulating the detailed sunmary of the information pertinent to this notification:
