REPORT ON TERMINATIONS TO KNOX FUSE BLOCKS

1. BACKGROUND:

This report is made after a request to the General Elec-tric Company by EBASCO to visit Louisiana Power and Light Waterford #3 at Taft, Louisiana en Dec. 13, 14, 1983 The purpose of the visit was to meet with the contractor, EBASCO Services, Inc., and the customer, Louisiana Power and Light, to review customer concerns over the electrical terminations made to Knox Cat.

  1. 525696 (M-6513410P-5) 600 volt, 60 amp fuse blocks mounted in metalclad switchgear.
2. SUPPLIER:

l 3 CONDITION / INVESTIGATION:

l i The wire terminations to Knox fuse blocks were inspected l

l in seven cubicles in six separate bus line-ups of metalclad switchgear. Four of the cubicles were in 1E equipments. All of the terminations in these cubicles

were found to be tight, the blocks firmly affixed to the

panels and the runs rigid and properly tied. Nothing

! was seen that would indicate these terminations were l

inadequate or unsafe for the circuits for which they l

l were designed. After inspecting the above seven cubi-cles two additional cubicles in two separate bus f

l line-ups were shown. It is not known whether these two cubicles were in 1E or non-1E equipments. In these two t m

mination was inserted in the lower clamp area of the connector on the Knox block. Although these termina- ,

tions were functional, they were not as secure as desirable. 'They were relocated to the upper clamp area of the connector, retightened and checked to ensure a good tight termination as left.

4. POTENTIAL SAFETY HAZARD:

Although these two terminations were found to be func-i tional and we know of no related field failures, the l

relocation significantly improved the security of the connection to the fuse block. This fuse block is a part of the circuit supplying tripping voltage to the cubicle breaker. A total loss of connection to this fuse would render the breaker unable to react to a fault condition when called upon. (This condition would be indicated by the loss of indicating lights).

5. EQUIPMENT AFFECTED:

Only 4160 volt and 6900 volt metalclad switchear equip-ment furnished to Louisiana Power and Light, Waterford 3 under requisitions 300-88690, 300-88691, and 300-88691-1 are affected. No other nuclear plants contain this Knox fuse block and consequently are not affected.

6. CORRECTIVE ACTION:

In written communications to EBASCO 12/16/83 and 1/17/84 and Louisiana Pcwer & Light on 1/25/84 we recommended that an inspection be made of the Knox fuse block wire terminations. Any instance of a single #14 tab terminal ' -

in the lower clamp area is to be relocated to the upper clamp area and re-tightened 3-5 ft/lbs. If the terminal is not visible but is firmly secured, or there are mul-tiple terminals in the lower clamp area, there is no need for relocation. We are awaiting the results of their inspection.

The purpose of the requested visit was to meet with the contractor (EBASCO Services Inc.) and the customer <

(LP&L), to. review concerns over the electrical connec-tions made to the Knox (Cat. #525696 G.E. Dwg. M-6513410 P-5) 60A fuse blocks (used in the breaker trip circuits of the 4160V Air Magnetic Metalclad Swgr) .

The subject fuse blocks were mounted on a vertical pan-l el across the width of the cubicles in the secondary

( front enclosure) area of the 4.16-350, 3000A V.L. AM Metalclad Swgr.

  • Dec. 13: At the afternoon, 3: 00PM, meeting, EBASCO personnel - C. kuiz and J. Grillo, reviewed the concern about the fuse block terminations. . .

Dec. 14: At the 9: 00AM meeting with nine L.P.&L. and two EBASCO personnel, L.P.&L. presented:

1. Their concerns regarding the type of connec-tions and methods used to connect the trip circuit wires to the Knox fuse blocks.
2. Questions on the logic for the 35 A fuse used in the trip circuit for the breaker.

! 3. I was reauested to review if a smaller fuse and fuse block could be used.

'O GENER AL $ ELECTRIC I responded to #2 by stating that the purpose for the 35 Amp fuse is'short circuit protection, not overload protection. We also intentionally size the fuse block (60A) to-prevent anyone inserting a smaller fuse. A smaller fuse (if blown) may lead to the 4160V bkr not able to trip when commanded. The combination 60 Amp block and 35 Amp fuse in the trip circuit has been our normal policy for many years. ,

I responded to #3 by stating that I would review this with other Design Engineers in Burlington.

Following this initial session we moved to various equipment locations to physically inspect the subject fuse block connections.

After this inspection we reconvened in the afternoon for additional review and comment.

OBSERVATIONS FROM INSPECTION:

During my inspection of the fuse blocks and their con-nections in the seven cubicles shown to me, I found all of the connections to be tight (each connection was pulled by me and the assigned electrician). In measur-ing the terminals I found the cross sectional area to be equal to or' larger than the cross sectional area of the wire. The blocks were firmly affixed to the panel and the runs were rigid and properly tied. I saw noth- .

ing that would indicate these terminations were inade-quate or unsafe for the circuit for which they were designed.

After inspecting the above seven cubicles, I was shown two other cubicles where single #14 tab terminals were

- inserted in the lower clamp area of the connector. 'Al-though these terminations were functional, they could be removed when pulled. We relocated them to.the upper clamp area of the connector, retightened and checked to ensure that it was a good tight connection as left.

This relocation clearly improved the tightness of the joint.

RECOMMENDATION:

Two instances were found where'a single #14 wire tab terminal was inserted in the lower clamp area of the fuse connector. This assembly, although functional, did not seem as firmly clamped as desirable. We therefore

l' GENER AL h ELECTRIC recommend that an inspection be made of Knox fuse block connections and any instance of a single #14 tab ter-minal in the lower clamp area be relocated to the upper clamp area and re-tightened 3-5 ft/lbs.

Note: If the terminal is not visible but is firmly secured, or there are multiple terminals in the lower clamp area, there is no need for relocation .

Since my return I have reviewed these two questions with other Burlington engineers. We continue to recom-mend that the design minimize the chance that the breaker trip circuit will be made inoperable by a blown fuse. If a smaller fuse block is used an operator could insert a smaller fuse. We therefore recommend no change. (Ref std policy (and other industry policies) along with the EBASCO Spec. 255-70, page 12, para. 5 (attached).)

INSPECTION REPORT 12/14/83 All Metalclad Swgr. was on line and energized. The cubicles L.P.&L. requested me to inspect were deactivated while we were there by opening the (DPST 30A) knife switch in the trip circuit.

The inspected line-ups by bus and cubicle number and status of fuse connections are as follows:

1. 6.9KV - BUS 3B1 - Cubicle #6 - Phila. Equip.

Fuse - top terminations - 1 #8-per pole - std insulated tab

Fuse - bottom terminations - 1 #14 per pole - uninsulated butt splice .125 dia x 56 lg.

All connections were pull tight and butt splices were full y inserted into connector, #8's were inserted in top area of connector.

2. 4.16KV - BUS 3A2 - Cubicle #9 - Burl. Equip.

Fuse - top terminations - 2 #8 per pole - insulated - ring terminals trimmed with holes remaining.

Fuse - bottom terminations - 2 #14 per pole std insulated ring terminals not altered.

All connections were pull tight and fully inserted in top area of connector.

3 4.16KV - BUS 3B2 - Cubicle #d - Burl. Equip. .

Fuse - top terminations - 2 #8 per pole - insulated - ring terminals trimmed to be a tab - no hole.

Fuse - bottom terminations - 2 #14 per pole - std insulated -

ring terminals not altered.

All connections were pull tight and fully inserted. Both

All connections were pull tight and fully inserted.

6. 4.16KV - 3B3-S - Cubicle #13 - Burl. Equip.

Fuse - top terminations - 2 #8 - insulated - ring term trimmed to be std spades.

Fuse - bottom terminations - 2 #14 - insulated std spades.

L.P.&L. then showed me two other cubicles where single //14 wires with std tab terminals were inserted in the lower clamp area of the connector. These wires could be pulled out of the connector.

I requested the tab to be inserted into the top clamp area of the connector and when tightened proved to be pull tight.

Dear Cesar,

Here are my comments to the three questions L.P.&L. posed in your Rapafax communication to me, received 1/10/84.

Q.1 The logic for using 35 amp fuses is given on pages 2 and 3 of my visit report, dated Dec. 16, 1983 Q.2 Our ' standard' design has not changed. We have been using the phenolic type fuse block since 1967 This fuse block accepts ring tongue type terminals ( AMP Pre-insulated Diamond Grip).

This fuse block, however, is rated 250V 60 amp not 600V 60 amp as required in your specification. We had recommended this 250V 60 amp pullout fuse in a letter to Mr. E. M.

Volpe, EBASCO Services from M. A. Sobert, G.E. New York.

See para. 6 of attached copy.

Note also, if this G.E. fuse block (0673D0515-422G004) were to be used as a replacement for the Knox block, all wire runs would have to be checked for sufficient length. This is due to the spacing (between the top and bottom terminals)

' on the existing Knox block being 6 3/4" whereas the G.E.

spacing is 2 3/8". The G.E. fuse block size is 4 1/4"W x 4 1/4"H x 3 3/8"D.

Q.3 A seismic qualification for this modification of your specific switchgear requires more than a casual quick look.

A study of specific locations and motions must be cor.sidered and an appropriate report issued, assuming no technical problems.

GENER AL $ ELECTRIC The cost for this analysis would be one to two (max) man-weeks of Design Engineering time. If other record changes are required, such as drawings or parts lists, etc., this must be specified and estimated also. If this is desired, an appropriate purchase order should be issued.

'le have mailed drawings for approval, P11y 211,1976, and would request that the following carments be confirmed prior to drawings being returned. We request clarification of the following points.

1. Specification Item 2.6 of Part 1 calls for 120 V 60 Hz breaker raise-lower power. We cannot furnish a suitable noter for this voltage, therefore,we woud suggest that we plan for 129 VDC elevating. This was done on previous Ebasco jobs - Homer City Generating Station and Arizona Public Service Cholla 2 and 3 Note too that such circuits are protected by fuses, not breakers.
2. The permanently located remote control station specified for breaker raise-lower nurposes will be a control switch in a box mounted on one end of each lineup, with a 40 foot cord attached, and which can be extended for plur,-in into any unit. One portable motor (125 VDC per Item 1 above) will be furnished per ' lineup, to be placed in the particular unit to be operated; and a mechanical latch will be supplied in each unit to " prop" out the clutch handle.

This handle would nomally initiate raise or lower operation, but

, the remote switch on this job takes over that function.

3 We would seek a waiver of Specification Part 1, Item 10.31 calling for visible check feature for CPT primary fuses. Such fuses are mounted on a rollout carriage whose front panel is approxinately 2 feet behind the unit door. Even with a glass insert in both door and rollout front cover it is fairly obvious that pri;".ary fuse status could not be detemined without opening the door.

Some is shown for 209 V - sone for 240 V - src units have no soccifications. We've assumed that ACB's are required in units controlling such motors, and have ordered such devices. In those units for which no arreter scale (for motor heaters) is given, we've listed 10A anmeters.

5. '1he control bus in our equipment is norrally in our standard wiring trouPh with other control wiring and we assume this does not violate the intent of Specification Part 2, Iten M.0. Can this sarc reasoning apply for the AC heater bus?
6. For Item if.h,5,6 of Part 2 of the specification, we plan to proceed as follows. We will furnish separate fuses for each of close and trip and elevatinc circuits. Closing fuses must be rated 15A for 125 VDC control and trip fuses are 3GA. We will furnish a 600 VAC -

'Xn VDC non-automatic ACB for disconnecting the purchascr's control sunnly of 125 VDC, a GE 'IU3801 PGO V, 30A knife nwitch for each breaker unit and 600 V control fuses for the close and trip circuits as mentioned above. Our standard and preferred reccmandation would be to sdoply 250 V fuses in pullout fuse assemblies and omit the individual latife switch; however, the 400 V fuse requirement rules out the pullout fuse assemblies.

7. Onecification Part 1 calls for chrtain overcurrent relay types.

Specification Part 2, Item 17.1 states that tir.e delay and instantaneous elements should be electrically separate. This is not consistent with the types specified. We have ordered specified types or their IAC S00 series equivalents since this seems to anree with CWD's.

8. We've assumed that our standard paint finish and procedure is OK except that color is to be Seafoam Green. This paint procedure has been accepted by Ebasco in previous , jobs.
c. We will furnish pre-insu]ated dianond grip, ring type terminals excent on SB-1 switches and certain other devices such as HGA relays with covers, where their use prevents cover installation.

10 On previous Ebasco . fobs, we have been permitted to supoly out standard Ef-16 lanps. Are these OK on this job?

11. Acastat 7022PC is listed in the vendor catalog as 1.5-15 second -

not .8-15 second time delay as on Ebasco CWD's.

12. Item 9.18 of Specification Part 2 calls for very soecial treatrent of fan coolinr: the Incoming Feeder breakers to uprate them from 2500 to 3000A. Because of the complexity of doing tnis where three fans are nomally used for basic forced cooling; and in order not to

/. ~ v GENER AL @ ELECTRIC . jeopardize basic operation of tl se breakers, we surr,est that Ebasco authorize the substitution of 3000A rated AM-13.8-1000 WA breakers for those units. l'i . Because of Item 12 above, we've ordered a Ground and Test Device for N1-7.?-900 feeder units only. If desired for Incoming Feeders, a senarate device would be required.

1 11 . Breaker limit switches (52'IOC) available for the Inccming Feeder breaker unit design mentioned in Item 12 have only 3 contacts closed for the lowered position. Since this meets the actual circuit requirenents shown 'on Ebasco CWD's for these units, they are assumed to be OK. Space in these units permits little or no deviation from this.

15 Ebasco should clarify the rating requirement for the resistor shown in PB/TS circuits. We're not sure of the intended scheme.

16. Please note the extra height requirement above the auxiliary ccr:partments.

Switchgear height is 115" but approximately 128" will be required to the top of the neutral. grounding resistors.

17 Ebasco should give us the reautred nameplate engravings for devices and units or advise the format to be followed in determining tdiat they should be.

l 30 Specification Item 15.1 of Part 2 calls for teminal points on teminal bea"ds to be marked with designations shown on Ebasco CMD's. Ebasco should clarify their meaning' We see no specific terminal number assignments on the Ebasco CND's.

l 20. Specification Part 2, Item 14.6 calls for one DC bus cutoff device,

! but Item 16.1 calls for bus entrance at each end of the switchgear.

We believe that these contradict each other at least as,regards the effectiveness of a cutoff device and we plan to provide for the DC entrance into customer unit 1 of each lineup.

DC bus renrenentation. We will ' viate from Ebasco CWD's' in one respect and that is in re. card to polarity mark location on CT's.

Our convention is to show polarity mrks toward the main bus in all cases. This insures consistent assembly routines in our factory and has no bearinr at all on electrical connection.

In addition, we do not plan to submit connection diagrams for approval. We see no direct instruction to do so and this agrees with our theory that it is GE's responsibility to mke such drawings agree with the e]ementary.

22. Et>asco speed letter of 19/19/73 to Nom Gundersen transmitted a list of device changes which. included an instruction to change circuit breaker control switch handles to brass type with built in lock suitable for locking and removinc: the key in "!'ormal" position. This seems to be inconsistent with and jeopaMize circuitrJ' callint" for the ability to trip the breaker via the local control switch and which apparently also is a two-handed oneration including operating of a pemissive push button. It would appear that locking of a switch in the "!Iormal" position prevents crernency trinning at the switchCear; and we hsve, for the present, ordered conventional pistol grip handles. If Ebasco insists that they be chanced, please docturent such requirement specifically.

23 We have not shown any detail for ;$hysical connection of cable bua-by others to our switchgear. Unless Ehanco can send information to us for such coordination, we will show our standard provision on the final issue of front views, and it will be up to the cable bus mnufacturer to adapt to this. We will also assum that actual physical make-un material for the connections will be the responsibility of others. Ebasco should advise or con"im our planned procedure.

In the 12/16/83 report on my visit to the L.P.&L.

Waterford 3 nuclear plant I made the following recom-mendation, "that an inspection be made of Knox fuse block connections and any instance of a single #14 tab terminal in the lower clamp area be relocated to the upper clamp area and re-tightened 3-5 f t/lbs. If the '

terminal is not visible but is firmly secured , or there are multiple terminals in the lower clamp area, there is no need for relocation ."

A sketch of the subject #14 terminal and the fuse block connector were also given.

After this inspection has been completed please notify our Mgr. of Design Engineering, Mr. Peter W. Dwyer, at this location. Please indicate the results of the in-spection,-which device and how it was connected, and -

the KV rating of the equipment along with the Bus and cubicle number affected.

s 44' GENER AL h ELECTRIC Question 4 - RESPONSE This was an accepted practice to modify wire termina-tions to adapt to the pressure-type connectors on the 600 volt Knox fuse block. Vendors supply this type of wire connector on the class of fuse blocks requiring higher voltage levels. Terminations were modified on all units sent for scismic testing and were checked before and after. Our Q.A. proced ures call for a test of all secondary control wiring per ANSI C37.20, para.

Question 5 - RESPONSE We terminate our wire connections to meet the provi-sions of ANSI Std. C37.20, para. 6.1 3 2.

We cannot really survey all standards to sea which al-low this type of construction.

I sincerely hope, (as you have indicated it would),
