Dear Mr. Keppler:
In response to the subject Bulletin, Public Service Company of Indiana, Inc. (PSI) has reviewed onsite radiograph sets of piping subassemblies supplied by ITT Grinnell, Kernersville, North Carolina. This encompassed a 100 percent review of the onsite radiograph sets for welds in shop fabricated ASME Code Class 1 and 2 piping subassemblies, regardless of wall thickness. The review resulted in the inspection of 69,048 radiographs with the applicable ASME Code penetrameter sensitivity being determined to have been altered on 129 of them. Of these 129 altered radiographs, acceptable weld quality on ninety-five (95) of them was demonstrated based on the equivalent or higher penetrameter sensitivity being discernible on the film or verifying the acceptability of a duplicate film. The remaining thirty-four (34) radiographs represented thirteen (13) welds on ten (10) piping subassemblies. Weld quality was demonstrated acceptable on eight (8) of these piping subassemblies by having radiography again performed on the welds in question and finding the resulting radio-graphs acceptable. The two (2) remaining piping subassemblies were rejected due to part geometry interfering with radiography of the welds.
For radiographs received after this review, Quality Assurance checklist attributes have been revised to include verification that the penetra-meter image shows no evidence of enhancement of the essential T-hole.
The possibility of ITT Grinnell, Kernersville, North Carolina, providing shop fabricated ASME Code Class 1 and 2 piping subassemblies with pipe wall thicknesses less than one half inch to other PSI suppliers is still being investigated.
