PURPOSE To describe the CNS Emergency Broadcast System (EBS) tone activeted radio malfunction reporting system and specify the procedure to be utilized for resolving such malfunctions. Responsibilities are also assigned herein for performing the requirements of the procedure. II. DISCUSSION EBS cone' activated radios have been placed in residences situated beyond l the effective range of the Early Warning System (EWS) fixed sirens in the Emergency Planning Zone (EPZ). These radios provide the primary means of notification for those residents who are not within fixed siren coverage. The operation and testing of these radios is described in EPIP 5.7.27 and accompanying attachments. Also included in EPIP 5.7.27, Attachment "N", is a copy of the operating instructions provided to each resident receiving an EBS tone activated radio. These instructions not only cover the operation and testing of these radios but also indicate a telephone number to contact for service or replacement of the radio should it fail to operate properly. This telephone is located at Cooper Nuclear Station. This telephone will 4 be manned 24-hours per day. Upon receipt of a call, the person answering the telephone will record the caller's name, telephone number, address, and pertinent information concerning their EBS radio malfunction. This infor-mation is forwarded to the CNS Emergency Planning Coordinator (EPC) where appropriate action is taken to correct the deficiency. III. REFERENCE MATERIAL A. I. PROCEDURE
A. Malfunction Report Receipt And Transcription. 1. A call any be made on a collect basis. 2. Data from reports of EWS tone activated radio malfunctions will be transcribed onto Attachment "A". 3. After filling out Attachment "A", forward it to the Energency Planning Coordinator at CNS. 4. In addition, a log will be maintained in the Emergency Planning Coordinator's Office to record pertinent data from each tone activated radio malfunction report. This log will serve as a mecharisa for tracking tha status of each report. B. Malfunction Correction. ( 1. Upon receipt of Attacliment "A", the Emergency Planning Coordinator or his designee will contact the resident reporting the malfunction l to arrange a suitable time to make a repair call. 2. The Emergency Planning Coordinator or his designee will call on the resident and determine if a malfunction has occurred. If the radio in question is not operating properly, it will be replaced. 3. After correcting the problem, the Emergency Planning Coordinator or his designee will indicate on Attachment "A" the nature of the problem and what action was taken to correct the malfunction. C. Malfunction Report Closeout. 1. Upon completion of Attachment "A" the Emergency Planning Coordinator or his designee will indicate in his log that the malfunction in question has been closed out. 2. Attachment "A" will be maintained in the Emergency Planning Coordi-l nator's Office as a record of malfunction reports made and correc-l i tive action taken. l IX. ATTACHMENTS A. ENERC'ENCY PLAN IMPLEMENTING PROCEDURE 5.7.27 EARLY WARNING SYSTEM ,~ I. PURPOSE The purpose of this procedure is to describe the Early Warning System within the Emergency Planning Zone (EPZ) of Cooper Nuclear Station. This system was set up to meet prompt notification requirements for Cooper Nuclear Station. It was also designed for response to any disaster; fire, flood, tornado, etc., where prompt notification of the public is desirable. II. DISCUSSION The Early Warning System consists of fixed sirens covering areas of high population density and tone activated Emergency Broadcast System (EBS) radios in the low population density rural areas. The fixed siren system is composed of ten pole mounted sirens located in or near the following towns: A. Nebraska: 1. Peru - one siren in town. 2. Brownville - one siren in town. (\\ 3. Nemaha - one siren in town. 4. Shubert - one siren in town. B. Missouri: 1. Watson - one siren near town. 2. Rock Port - one siren in town. 3. Phelps City - one siren in town. 4. Langdon - one siren near tcwn. 5. Nishnabotna - one siren near town. 6. Intersection of US-136 and I one siren near this intersection. -1 .. _ _ _ ~ D The EBS tone activated radios are in residences located within the rural EPZ areas outside the hear'ir.g range of the fixed sirens. These radios are pretuned to the Primary Emergency Broadcast Stations (KFAB-1110 kHz Omaha, within the Nebraska EPZ and KFEQ-680 kHz St. Joseph. within the Missouri EPZ) and are automatically activated when the EBS ~ station sounds the EBS cones. These radios also have battery backup power. In addition to the primary means of notification, fixed sirens and tone activated radios, a backup system consisting of vehicle mounted mobile siren units is available. This Mobile Siren System is comprised of mobile units following designated routes. Mobile sirens have been provided to each volunteer fire department within these areas to cover the number of routes specified below. ~ A. Nebraska: 1. Peru - four routes. 2. Brownville - four routes. 3. Nemaha - four routes. 4. Shubert - two routes. 's - B. Missouri: 1. Rock Port - seven routes. 2. Watson - four routes. In addition, the Richardson and Nemaha County Sheriff's personnel also have designated notification routes. See Attachment "D" for maps of the designated routes. l The nobile sirens are Whalen Engineering Company Model 370 PA speakers coupled to a Whalen WS-295 power amplifier. s IV. PREREQUISITES ^ A. Prompt notification to the public is deemed necessary for implementa-tion of protective action guides'. This will normally be determined by state and local authorities in accordance with their respective Nuclear Power Plant En;ergency Response Plans. The CNS Emergency Director will recommend protective actions to these authorities in accordance with EPIP 5.7.20, Protective Action Guides, or in severe situations, may activate the Early Warning System in accordance with EPIP 5.7.6, Notification. V. LIMITATIONS ThekarlyWarningSystem,inadditiontoitsuseasameansofprompt A. notification in the event of an incident at Cooper Nuclear Station requiring a protective action, may be used to warn the public of attack, tornado, flood, fire, or any disaster requiring prompt noti-fication of the public. 1 VI. The number of operable mobile sirens at each location shall meet or exceed the number of mobile siren routes assigned to each location. Spare mobile sirens may be maintained at each location. 1. Nebraska: a. Peru - four routes with three mobile units ' mounted on fire trucks and one universal mount mobile siren unit within the Fire House. b. Brownville - four routes with three mobile units mounted on fire trucirs and one universal mount mobile siren unit within ~ the Fire House. Nemaha - four route's with one mobile unit on a fire truck and c. three universal mount mobile siren units within the Fire House, d. Shubert - two routes with two mobile units within the Fire House. e. Indian Cave State Park - one mobile unit mounted on Park Ranger vehicle. f. Nemaha County Sheriff - two routes with two mobile units ( mounted on Sheriff's Department vehicles. g. Richardson County Sheriff - one route with one mobile unit mounted on Sheriff's Department vehicle. .2. Missouri: a. Rock Port - seven routes with four universal mount mobile siren units stored within the Fire House and three units installed on fire vehicles. b. Watson - four routes with one universal mount mobile siren unit within the Fire House and three units installed on fire vehi-cles. D. Conusunications equipment Installed for the backup mobile siren system includes base and mobile two-way radio units to the respective County Sheriff Departments for the following, if equipment did not already exist before installation of the EWS: 1. Nebraska: a.- Peru - one base and one mobile. b. Brownville - one base and one mobile.

I. PROCEDURE
A. Fixed Sirens. 1. Activation. a. Activation of the fixed sirens will be accomplished by the County Sheriff in the county which the siren is located: 1) Nemaha County Sheriff activates the fixed sirens in Peru, Brownville, and Nemaha (Nemaha County Sheriff has the capability to also activate the Shubert fixed siren). 2) Richardson County Sheriff activates the fixed siren in Shubert. p 3) Atchison County Sheriff activates the fixed sirens in Rock Port. Watson, Phelps City, Nishnabotna, Langdon, and US-136 and I-29 intersection. b. The Sheriffs will be requested to sound the fixed sirens in their respective areas by the local and/or state emergency response authorities. These authorities will have responded to a Cooper Nuclear Station emergency in accordance with their j respective state and local emergency response plans. In a i severe situation, the Sheriffs may be reauested to sound the sirens by the CNS Emergency Director if immediate protective action. is deemed necessary. The CNS Emergency Director will then contact the appropriate Emergency Broadcast System (EBS) i station (s) to activate the EBS. To activate the EBS the Emergency Director must telephone KFAB and/or KFEQ and provide i an authentication code to the station (s) in order - that radio [ station personnel may be assured the Emergency Director's call l 1s legitimate. The authentication codes are located in the j glass case in the Shift Supervisor's Office in the Centrol Room. The glass must be broken to obtain the code. Once the authenticity of the call is established, the Emergency Director will deliver the appropriate preformatted message to the EBS station (s) in accordance with EPIP 5.7.6 Notification. r' 2. Testing by an actual activation of the fixed sirens will be accom-plished in accordance with the following schedule: Nemaha County - third Sa'turday of each month, a. b. Richardson County - fourth Friday of each month, c. Atchison County - first Saturday of each month. 3, Maintenance or repair. a. If repair of the fixed sirens is required, the Cooper Nuclear i Station Control Room personnel ((402) 825-3811 or 825-6811) I' will be notified and will initiate necessary repair action, logging, and notification. -Note: See Attachments "A", "B", and "C" for Sherif f's proce-dure to activate the fixed sirens. B. EBS Tone Activated-Radios. 1. Activation. a. Activation of the EBS radios are accomplished by the primary EBS transmitter stations; KFAB-li10 kHz Omaha for Nebraska EPZ, and KFEQ-680 kHz St. Joseph for Missouri EPZ.. When these stations activate their emergency broadcast, a dual tone is sounded which turns en the EBS tone activated radio receivers - s and then the emergency message is broadcast. The EBS tone activated receivers stay on, continuing with the EBS station's program format until reset to automatic. b. The primary Emergency Broadcast station will be requested to broadcast an EBS message by local or state emergency response authorities. These authorities will have responded to a Cooper Nuclear Station emergency in accordance with their respective state and local emergency response plans. In a severe situa-tion, the stations may be requested to broadcast an EBS message by the CNS Emergency Director if immediate protective action is deemed necessary. To activate the Emergency Broadcast System (EBS) the. Emergency Director must telephone KFAB and/or KFEQ and provide an authentication code to the station (s) in order that radio station personnel may be assured the Emergency Director's call is legitimate. The authentication codes are located in the glass case in the Shift Supervisor's Office in the Control Room. The glass must be broken to obtain the code. Once the anthenticity of the call is established, the Emergency Director will deliver the appropriate preformatted message to the EBS station (s) in accordance with EPIP 5.7.6, Notification. ,I ^ j 2. Testing. a. Once per week the primary EBS stations test their EBS systen uhich activates the tone activated radios and proceeds with the message. In the event. of an actual emergency appropriate informatica would be broadcast at this time. Thus, individuals with EBS tone activated radios will have their radios automat-ically tested once per week. A test button is provided on each radio to check the radio by bypassing the automatic activation function to verify operability. In addition' the EBS activated radios have a battery test button and indicator light to check the battery backup power source. 3. Maintenance or repair. I a. Should the EBS activated radio fail any.of the above tests, ~ malfunction, or need asintenance or repair, a telephone number is posted on the radio for the individual to call to initiate corrective actions. See Attachment "N" for description of material attached to the radio. Reporting and correction of tone activated radio malfunctions shall be conducted as spec-ified in EPIP 5.7.27.1 EBS Tone Activated Radio Malfunctions. C. Backup Mobile Sirens. 1. Activation. 3 a. The backup mobile siren system may be utilized at the decision of local or state emergency authorities on a contingent basis to augment the fixed sirens and EBS tone activated radios. In l a severe situation, this system may also be activated by the Emergency Director. I b. Upon the decision to activate the backup mobile siren system, Volunteer Firemen will be called to their respective Fire Houses by the sounding of their local fire siren. The County t l Sheriff may activate this siren by radio encoder or he may contact the local Fire Chief who will activate the siren himself. l' c. The Fire Chief or Senior Firemen will contact the County l Sheriff.by radio or telephone and request information. If the l backup ~ aobile siren system is to be activated, Firemen will place the mobile siren units en vehicles (if not already installed), perform an operational check, and drive the routes as outlined in Attachment "D". Each Fire House has a procedure outlining these. steps. Attachments "E" through "L" contain -these procedures. Mobile sirens at the Fire Houses will normally be tested during a. the scheduled Fire Department meetings. Testing and operabil-ity of the sirens will be verified during regularly scheduled Quality Assurance audits. 3. Maintenance or repair. a. Responsibility for. maintenance and repair of mobile sirens rests with Nebraska Public Power District. If a mobile siren is found to be inoperable or in need of repair, the Cooper Nuclear Station Control Room personnel ((402) 825-3811 or 825-6811) should be notified. ATTACHMENT "A" ENERGENCY PLAN IMPLEMENTING PROCEDURE 5.7.27 EARLY WARNING SYSTEM i NEMAHA COUNTY SHERIFF - ACTIVATION OF EARLY WARNING SYSTEM In the event of an emergency at Cooper Nuclear Station or any civil disaster which may require immediate alerting of people frca the towns of Brownville, Peru, Shubert, and Nemaha, the following procedure will be utilized: s 1. Sound the alert tone for sirens in the designated areas (use code near con-trols to alert individual sirens or all sirens sounded at once). Sound the sirens for several three-minute cycles. Note:.The Shubert siren may also be sounded from Richardson County Sheriff's Office. 2. The sounding of these sirens alerts residents in these communities to tune to the Emergency, Broadcast Station, KFAB-1110 kHz, for emergency information. Rural residences within the Emergency Planning Zone will be alerted with EBS tone activated ' adios when the EBS message is broadcast. 3. Contact the Indian Cavo Parst Ranger by radio or telephone and request him to alert visitors. 4. If activation of the backup mobile siren system is deemed necessary: a. Establish communications with the local Fire Chief in the siren area by (' telephone or radio (Peru - radio, Brownville - radio, Nemaha - Fire Chief radio or telephone). b. The Fire Chief will dispatch the Firemen with mobile sirens on predesig - nated routes. These achile sirens have public address systems and will be used to alert residents along their routes to tune to the Emergency i Broadcast Station. I c. Dispatch two Sheriff's Officers to cover the Nemaha County Sheriff's mobile siren routes. d. The Fire Chief is to report back when the routes have been completed. l e. Once notified by the respective Fire Chief and Sheriff's personnel that j all Early Warning routes have been completed, the Nemaha County Sheriff l will-relay this information to the Nemaha County Emergency Operations Center. If activation of the Backup Mobile Siren System is deemed necessary: a. The Fire Chief will dispatch the Firemen with mobile sirens on predesig-( nated routes. These mobile sirens have public address systems which will 4 be used to alert residents along their routes to tune to the Emergency Broadcast Station. c. Dispatch Officer to cover Richardson County Sheriff's mobile siren route. i d. The Fire Chief is to report back when the routes.have been completed. e. Once notified by the Fire Chief and sheriff's personnel that all Early i Warning routes have been completed, the Richardson County Sheriff will relay this information to the Richardson County Emergency Operations j-Center. Testing: Activate the fixed sirens in Shubert at 1630 on the fourth Friday of each month. Activate the siren for 15 to 20 seconds and then cancel. Contact Shubert to verify the siren was sounded. Log this activation and verification of sounding in the Dispatcher's Log Book. To provide an efficient means for evacuation of non-essential personnel j from isolated areas or from the plant site in its entirety. B. Io provide a definition of the duties and responsibilities of designated supervisory personnel associated with site evacuation. II. DISCUSSION l A. In the event of an emergency situation at CNS, it may be desirable to minialze the number of non-essential personnel on site. If the emer-gency involves a radiological release or the potential for a release, then evacuation of non-essential personnel is desirable, or may be required, to minimize exposure to radioactive material. B. The organization of this procedure provides guidance in three distinct areas: 1. Determination of the need for site evacuation. 2. Determination of the evacuation route and assembly area. l 7 3. Conducting the evacuation and subsequent assembly, monitoring and release of persc tel in an orderly manner. The first two of these areas are provided in the Section A., Emergency Director. The latter is provided in the Section B., Evacuacion Coordi-nator. C.. This procedure is intended to apply to evacuations where persons may receive abnormal external exposure and/or. persons or automobiles may be contaminated. It is recognized that in the event of an energency it 3 may be desirable to send persons home before they are exposed to significant radiation and/or contamination levels. An orderly sequence l _ of dismissal should be-given by the Emergency Director-D. Personnel on site will assemble at their designated assembly areas following sounding of the emergency alarm in accordance with EPIP 5.7.10. Personnel Assembly And Accountability. V.. LIMITATIONS A. A SITE AREA EMERGENCY or GENERAL EMERGENCY has been declared in accor-dance with EPIP 5.7.1 Emergency Classification. B. This procedure assumes the emergency occurs during normal working hours when the greatest numbers of non-essential personnel are on site. It .is.also applicable, however, during normal off-duty hours, weekends, .and holidays. C. MPC is the Maximum Permissible Concentration as defined in Column 1 Table ~ I, ' Appendix B to 10CFR20.. This calculation will allow 200 1 1 1MPC/Nrs which conservatively limits internal exposure. -This criteria is based on personnel not wearing respiratory equipment. + VI. ' PRECAUTIONS A. B. Local areas within the station will be evacusted if any of the following f, , conditions exist: 1. Unscheduled alarms from any of the area or airborne' radiation monitors. 2. Detection of excessive radioactive surface contamination levels. 3. Other emergency conditions, such as fire, that may endanger human life or health. C. Monitoring points between CNS and the assembly areas may need to be set up depending upon the emergency condition. This is to prevent the apread of contamination. VII. EOUIPMENT A. None. VII
I. PROCEDURE
A. Erergency Director. 1. Assess the need for, or the potential need for, evacuation of non-essential personnel. Attachment "A", Evacuation Criteria, provides guidance in making this determination for radiological emergencies. Generally evacuation of non-essential personnel will be. conducted for incidents at the SITE AREA EMERGENCY or GENERAL EMERGENCY Classification Levels regardless of whether a release is oc.murring. 2. Appoint an Evacuation Coordinator to supervise the evacuation. 3. Designate the evacuation route and off-site evacuation assembly area. Attachment "B", Evacuation Route Determination, provides guidance in this determination. 4. Direct the notification of the Nemaha County EOC or the Nemaha County Sheriff's Office of the evacuation. a. Notification of the intent to evacuate personnel includes:
- 1) The intended evacuation route.
- 2) The designated off-site assembly area, if other than the AEOF.
3)' The approximate number of personnel and vehicles to be evacuated.
- 4) The anticipated need for any decontamination of personnel or vehicles will be coordinated with the Nemaha County Decontamination Center.

=. g-s , 5. Determine the personnel to be retained on site and personnel to be assigned-to off-site emergency response locations or relief shift duty. In general, personnel other than operations retained on site will have chemistry, radiation protection, or maintenance assign-ments for the site emergency organization. 6. Provide evacuation instructions to the Evacuation Coordinator. i These instructions should include: a. Evacuation route. b. Off-site assembly area if.other than AEOF. c. Anticipated vehicle or personnel monitoring and decontamination requirements. d. Arrangements with off-site response agencies. e. Personnel to remain on site. B. Evacuation Coordinator. 1. The basic responsibilities of the Evacuation Coordinator are as follows: a. Coordinate with the Emergency Director to determine the condi-tions of the emergency and the evacuation plan. b. Provide information on the number of vehicleo and personnel involved in the evacuation and any further personnel account-ability required at the site prior to the evacuation. -t c. Providing required on-site traffic control measures such as: 1) Notifying Security of the intended evacuation.
- 2) Sequencing the departure of personnel to avoid congestion, d.. Establish monitoring points along the evacuation route, where and if appropriate.
2. Contact the Chemistry and Health Physics Coordinator and assure that qualified monitoring personnel are dispatched to the monitoring points (either. f rom on-site or of f-site). Generally one monitor for each of the areas will be utilized. 3. Provide instruction for personnel evacuating. These generally are:

b. Personnel without transportation should obtain a ride with a driver in their assembly area. i c. At the off-site assembly area, each assembly area evacuation - leader should assure that personnel are accounted for and remain to be monitored and cleared before release. .C. Activities At The Monitoring Points. 1. Upon arrival at the monitoring points the monitor (s) shall begin a program of surveying personnel and vehicles for contamination. The results are to be recorded on Attachments "C" and "D". Personnel and/or-equipment (vehicles, etc.) that are contaminated in excess of the limits presented in Attachment "E" must be decontaminated per EPIP 5.7.13. Decontamination of personnel and/or vehicles with contamination levels less than those presented in Attachment "E", but higher than two times background will be at the discretion of the radiation monitor (s) at the evacuation area. The overall results are to be reported to the Emergency Director. 2. If parking lots'are utilized as the monitoring points and the lots cannot be completely cleared prior to arrival of evacueea, all vehicles which were originally in the lot will be surveyed before being allowed to leave. 4 3. As a minimum, the names and addressee of any evacuees suspected of having received doses in excess of 250 mR and those requiring any decontamination shall be obtained before the persons are allowed to leave the monitoring point. 4. In general, personnel shall be given permission to leave the monitoring point and proceed to the assembly point only af ter the following conditions are mets The person and his vehicle have been surveyed or a sufficient a. number of persons in the group have been surveyed in order to determine that radioactive contamination is not a factor. b. Self-reading dosimeter results have been recorded and the names of exposed persons recorded. .c. The above-results have been reported to the Emergency Director ~ for his evaluation.. g D.' ' Evacuation of The Central ' Alarm Station. 1. In the event the Central Alara Station (CAS) becomes uninhabitable. as determined by on-site radiological monitoring, security personnel shall be evacuated from CAS. The Aubum National Guard Armory has been designated as the Alter-nate Assembly Point to be used in the case that for any reason the Security Building Auditorium cannot be used. 2. If a site evacuation is required, the Armory will be used as an assembly point with monitoring points selected depending upon the emergency conditions. 3. If the on-site decontamination facility is not available, or because of time constraints during an evacuation the facility cannot be used, or interim monitoring points cannot be used, contaminated personnel will be directed either to the National Guard Armory or to the Nemaha County Decontamination Center. "*- s EVACUATION CRITERIA The decision to evacuate any, or all, of the personnel covered by this procedure shall be made by the Etergency Director. Personnel shall not evacuate an assembly area unless instructed to do so by the person in charge in the area, and this individual shall, in turn, receive his instructions' from the Evacuation Coordinator who, in turn, receives instructions from the Emergency Director. A. CNS Contract Personnel. Visitors, Clerical Station Personnel. In keeping with as low as reasonably achinvable philosophy, personnel who are not contributing substantially to emergency response actions (which generally includes visitors, contractors, and clerical personnel) should be . evacuated if such action can prevent significant exposure, provided that trained personnel are available to conduct the evacuacion and can be spared for this task without jeopardizing emergency mitigation activities. 1. Whether or not the emergency can be mitigated prior to reaching a dose of 500 area or 200 MPC/ Hrs non-essential personnel (nonradiation worker criteria). 2. If personnel involved are rot'immediately essential for handling the emergency, they should be evacuated at levels near the low and of each range to minimize their doses. 3. Any time personnel are to be evacuated, the dose expected during evacuation must be weighed against that expected if the person is not evacuated. In some cases, evacuation may result in a higher dose than could be received if the individuals remained in a shielded or protected area. The specific route to be followed shall be determined by the Emergency Director on the basis of wind direction, dose rates, and other pertinent factors existing at the time. A. Description Of Routes. 1. Northern Route through Brownville. The preferred route if evacuation from the site is required is out the access route to the County road, north on the county road to Brownville, then west on U.S. liighway 136 to Auburn.
