This letter refers to the violation cited in the subject inspection report.
The violation states:
Technical Specification 6.8.1 requirus that applicable written procedures recommended in Appendix A of Regulatory Guide (kG) 1.33, Revision 2,1978 shall be established, implemented and maintained.
Procedure FNP-0-AP-16, Conduct of Operations - Operations Group, Revision 21, Section 3.2.9.2 requires plant operators to take timely and proper actions that will not create a hazard to safe operations to the facility.
Contrary to the above, on August 2, 1991, at approximately 12:01 a.m. the unit operator in concert with the system operator, without verifying the function of an electrical circuit opened the circuit breaker which supplies power to the "lC" reactor coolant pump breaker indication circuitry.
Opening this breaker caused a reactor coolant pump breaker open signal to be generated in the reactor protection system.
This resulted in an unplanned automatic reactor trip.
This is a Severity Level IV violation (Supplement 1).
Admission or Dental The above violation occurred as described in the subject report.
Reason for Violation This violation was caused by personnel error.
The Plant Operator granted permission to reset the breaker which provides power to the IC RCP breaker position indication contact without verifying the function of the breaker or determining the cause of the apparent tripped condition.

A caution label which reads " Opening this breaker may cause a reactor trip" has been placcd on the four associated breakers in Unit ' and four associated breakers in Unit 2.
