NRC Generic Letter 89-04 requires licensees who revise their IST programs to submit those revisions to the NRC (References 1 & 2). Attachment I is Revision 4 of the IST Program, which reflects various olant modifications, changes in Emo,guncy

submits the revisod IST Program in accordanco with the requiromonts in NRC Gonoric 1

NRC Genoric Lotter 89-04 also requires licensees to submit all open roliof requests for NRC review (References 1&2). Attachment 11 provides the status of all rollof requests that are included in Revision 4 of the IST Program. They aro categorized as opprovod, pro-approved (as defined Reference 1), withdrawn by the Authority, or requiring NRC review. The Authority requests that the NRC review the opon relief 1
