This 11etter is. to--confirm my report by phone to Mr. Seidel of a reportable l occurrence under our Technical Specification 3.5.1, " Radiation Monitoring". -Un 7oJune 1991,-the reactor'was operated for 30 minutes at l'MW without-the 1 1 i Facility Air-M(nitors in' operation. I was the Senior Reactor Operator who ~
- startedsup the reactor,. and I was previously aware that the Facility Air Monitors were.not in operation.
I did not become aware of the violation until I was informed - of the problem by the Reactor Supervisor on Monday morning, '10 F ~' June ' 1991 - Mr. Donald. Feltz,- Director of the Nuclear Science Center, has removed me from watch standi_ng duties as a result -of this incident. A final report -of 'all the--findings. concerning this incident will be forwarded to your ' office upon ' its completion.
