Response to NRC__ Concern About Radiation Protection

Dear Mr. Martin:

Illinois Power (IP) has reviewed the concern contained in the NRC letter to IP from Roy Caniano dated November 30,1994, rescrding an incident in the Radwaste Operations Center (ROC) at Clinton Power Station (CPS). Tids concern was reviewed by an organization independent of the affected department.

The event de_serihed in the NRC letter was documented on CPS Condition Report (CR) 1-94-07-006 in July,1994. Similar concems to the one contained in the NRC letter weie also soccived by the CPS Quatty Report system. A summary of the investigation results is provided below.

On June 28,1994, a member of CPS management discovered, what appeared to be, an individual sleeping in the ROC. In discussions with this management staff member, as part of the investigation, it was determined that the facts were inconclusive as to whether or not the individual was actually n,leeping. When intes viewed, the Radwa.ste operator involved denied that he was sleeping. The operator involved in this incident was not assigned to any watchstation on the day mentioned above.

In light of the absence of conclusive evidence, the individual wcs counseled on the less than acceptable performance demomtrated by giving the peiception oraleeping. No other corrective actions were taken or are planned for this incident.

The NRC letter referred to this being the second instance of sleeping on thejob for the individual involved. A review of the individual's personnel record documents that this individual did receive disciphnary action in January,1990 for a similar event.

_About Radiation Protection

Dear Mr. Martin:

On December 28,1994, Illinois Power (IP) issued letter U-602386 in response to the concern contained in the NRC letter to IP from R. J. Caniano dated November 30, 1994. The NRC letter requested that IP submit infonnation regarding a concern about an individual sleeping in the Radwaste Operations Center (ROC). This letter supplements IP letter U-602386 and provides additional information about the IP investigation of the incident as requested by R. J. Caniano ofyour staff on January 5,1995.

The independent investigation of the concern was performed by the Licensing Department supervisor of regional regulatory mterface. The investigation included reviewing the condition report (1-94 07-006) that was initiated in July 1994 and contains the initial investigation of the sleeping incident, reviewing tv.o employee concerns (Quality Reports 1934 and 1938) related tc the incident, intersiewing the management staff member (the manager of nucl:ar training) who discovered the inattentive individual, and interviewing Operations management regarding the investigations that were conducted and the corrective actions taken when the incident was ini:ially identified The manager of nuclear training related that after entering the ROC, and during discussions with the on duty ROC operator, the manager noticed an individual in an adjacent room that was in a very relaxed position. When the manager questioned the ROC operator about the identity of the individual, the on-duty ROC operator responded in a loud voice, naming the individual. At this point, the relaxed individual moved in an apparent startled manner. The manager was unable to declare positively that the individual was sleeping. The manager brought the situation to the attention of Operations management fer appropriate action.

The assistant director of Operations related that he interviewed the individual because the evidence that the individual was sleeping was inconclusive. During thc interview, the individual reportedly denied that he was >1ceping. On the basis of the interview, and the fact that the individual was not standing watch at the time of the i

incident, the individual received verbal counseling about the expected appropriate behavior. The director of Operations related that he also interviewed the individual, and the individual again denied that he had been sleeping. Again, due to the lack of evidence, the indMdual only received verbal counseling.

Quality Reports 1934 and 1938 were reviewed but did not provide any additional conclusive information about this incident The indMdual has admitted bleeping while on duty during a previous incident. A review of the individual's personnel record identified that he was involved in a similar incident while on duty in the ROC on January 17,1990. Unlike the most recent incident -

ofinattention, when the 1990 incide.nt occurred, the individual was. standing watch in the ROC. During the investigation of the 1990 incident, the individual admitted that he had been sleeping while standing watch. Disciplinary action for that incident included time off without pay.

A copy of condition report 1-94-07-006 is enclosed with this letter as requested by Mr. Caniano. A search of corrective action tracking data bases did not identify other incidents of personnel sleeping on the job.
