The following is a transcription of the violation as cited in the Notice of Violation (NOV):

" During the NRC inspection conducted on September 19 - 23,1994, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and

Procedures for NRC Enforcement Actions," 10 CFR Past 2, Appendix C, the violation is listed below:

hianual, Revision 12, requires, in part, that activities affecting quality shall be prescribed i
by documented procedures.. and shall be accomplished in accordance with these procedures.
Paragraph 11.2 ofMagnetic Particle Examination procedure No. lit-V-505, Revision 3, states that, broad areas of particle suspension accumulation are unacceptable, and those areas shall be cleaned and reexamined.
Contrary to the above, on September 21,1994 a magnetic panicle examiner was observed performing a magnetic examination on the pressurizer skirt weld without removing residual buildup of h1T particles left by the previous overlapping inspection in the other direction.

RESPONSE TO VIOLATION (50-424:425/94-24-01)
Admission or Denial of the Violation:
This violation occurred as stated in the notice of violation.
Reason for the Violation:
This violation was attributed to a cognitive personnel error on the part of a contracted inservice inspection (ISI) technician that failed to follow the requirements of procedure hfT-V-505 while performing a magnetic particle (hit) examination on a vertical weld on the pressurizer skirt.

ENCLOSURE VOGTLE ELECTRIC GENERATING PLANT - UNIT 1 REPLY TO A NOTICE OF VIOLATION NRC INSPECTION REPORTS 50-421:425/94-24 Corrective Steps Which IInve Been Taken and the Results Achieved:
The involved individual has been counseled on his failure to adhere to precedural requirements and was immediately restricted from performing any other MT examinations.
All contract ISI technicians were briefed on this violation and the importance of procedural adherence. It was stressed that the responsible GPC supervisor should be contacted if there is any confusion or question regarding ISI examination techniques.
Corrective Steps Which Will He Taken to Avoid Further Violations:
For future refueling outages, site specific procedure reviews will be conducted with the contract ISI technicians prior to the start ofISI activities. These reviews will cover procedural requirements and the importance of adhering to these requirements.
Date When Full Compliance Will He Achieved:
Full compliance was achieved on September 21,1994 when the pressurizer skirt weld was successfully re-examined using the proper techniques as delineated in procedure MT-V-505, paragraph 11.2.
