Please find enclosed, the Waterford 3 Quality Assurance Program Manual updates generated for the period December 16, 1992 through October 7, 1994.
As you recall, the Waterford 3 Quality Assurance Manual was submitted (W3F192-0218) on December 16, 1992 jointly with the Final Safety Analysis Report (FSAR) Update in lieu of deleted Chapter 17.2 of the FSAR.
Also enclosed is a list of chapters / appendices revised with a brief statement of the reason for the revision.
The current submittal is being made in accordance with 10 CFR 50.54(a)(3). All pages of each of the affected chapters are being submitted (instead of only the changed pages) due to the extent of the affected pages and pagination shifts.

Revisions to Quality Assurance Program Manual from December 16, 1992 through October 7, 1994 (Page 1 of 2) a)
Chapters 1, 2, 6, 7, 10, 15, & 16 Revision 2.0, Chapters 3, 11, & 14 Revision 0.0; and Chapters 4, 5, 8, 9, 12, 13, 17, & 18 Revision 1.0 were revised to reorganize Waterford 3 site organization to increase span of control; to delete mention of the Nuclear Operations Management Manual (NOMM) which was deleted when contents were incorporated in other procedures; and to show that definitions are contained in Appendix C.
Chapter 7 Revision 2.0 was also revised to clarify that product acceptance may include pre or post installation testing. Chapter 12 Revision 1.0 was also revised to clarify purpose of procedure; add definition of Measuring and Test Equipment; provide clarification on calibration standards, recall, test, and documentation; and identify devices not requiring calibration and control measures.
Chapter 13 Revision 1.0 was also revised to define that USNRC licensed containers will be leased from USNRC licensed suppliers.

Chapter 16 Revision 2.0 was changed to reassign responsibility to review corrective action documents to determine if the identified condition represents a significant condition adverse to quality from the General Manager - Plant Operations to the QA Manager for Condition Reports.

Appendix C Revision 2.0 was revised to reflect the definition of Peer / Maintenance Inspector, Special Test, and Measuring and Test Equipment as contained in IEEE 498-1980 to include the exclusion of applicability of permanently installed operating equipment and test equipment used for preliminary checks, d)
Chapters 1, 2, 7, 10, & 15 Revision 3.0 and Chapters 4, 8, 13, 17, & 18 Revision 2.0 were revised to reflect responsibilities at a higher management level, instead of at a department level as was the current practice; and to reflect the Vice President, Operations Support taking over responsibility for the Echelon based supplier Quality Assurance functions.

Chapter 2 Revision 4.0 was revised to more accurately reflect the September 30, 1992 requirement of 10CFR50.71(e) (4) regarding when revisions to the QAPM must be submitted to the NRC.

Chapter 15 Revision 4.0 and Chapter 16 Revision 3.0 were revised to add creation of Condition Review Board (CRB), definition for Adverse Condition, & requirement that a root cause analysis with action to prevent recurrence be conducted for all identified significant adverse conditions, also to reassign some responsibilities from the General Manager Plant Operations (GMP0) to the Director, Nuclear Safety.

Appendix C Revision 3.0 was revised to reflect new or revised definitions for Abnormal Condition, Adverse Condition, cause, Corrective Action, Discrepancy, Nonconforming Condition, Special Scope, Significant Adverse Condition, and delete root cause.

Chapter 1 Revision 5.0; Chapter 3 Revision 1.0; Chapter 5 Revision 2.0; Chapter 18 Revision 3.0 and Appendix A Revision 2.0 were revised to duplicate & insert the requirements containeo in Section 6.5 of the Waterford 3 Technical Specification.

Chapters 1 & 2 Revision 5.0 & Chapters 6 & 18 Revision 3.0 were revised to Change the Title of Entergy Operations Management Manual (E0MM) to Nuclear Management Manual, and E0MM Vol. IV to Waterford 3 Management Manual.
Chapter 2 Revision 5, Attachment I was revised to allow completion of quality related reviews of Design Engineering procedures by other than Quality Assurance personnel, j)
Appendix C Revision 4.0 was revised to update the definition of Conditions Adverse to Quality.

Chapter 2 Revision 5.0 was revised to include the Design Engineering Administrative Manual (DEAM) as part of the Waterford 3 Document Hierarchy
