SPECIAL REPORT 90 04 RESULTS OF FIFTH STEAM CENERATOR TUBE INSERVICE INSPECTION This report is submitted in accordance with Technical Specification (T/S) 4.4.5.5 b which states:
"The complete results of the steam generator tube inservice inspection shall be cubmitted to the Commission in a Special Report purst.nt to Specification 6.9.2 within 12 months following the completion of the inspection.
This Special Report shall include:
(1) Number and extent of tubes inspected, (2) Location and percent of wall thickness penetration: for each indication of an imperfection, and (3)
Identification of tubes plugged."
In October 1990, with the plant in Mode 6 Refueling for the fourtl-refueling outage, the fifth plant Stec.1 Generator (S/G) Eddy Current Inservice Inspection was performed, S/G's
'A' and
'D' were inspected. The inspection was performed by Westinghouse Nuclear Services Division (NSD) and their formal results are detailed in the Eddy Current Examination (ECE) which is submitted with this Special Report.
Although the Westinghouse NSD ECE provides details on the inspection, the results are summarized below and are numbered to correspond with the information requested by T/S 4.4.5.5.b:

One hundred percent of the unplugged tubes or five thousand six hundred fifteen (5615) tubes were tested full length using the bobbin coil technique. The technique utilized frequencies of 630, 300, 150, and 30 kilohertz. Also, in Steam Generator
'A', 223 tubes were examined with a 0.580 motorized rotating pancake coil (HRPC) probe in the hot leg tube sheet region. All tubes examined with the MRPC probe in Steam Generator
'A' had no detectable discontinuities.
There were four (4) tubes plugged in Steam Cencrator
'A' for anti vibration bar indications above 40 percent through wall depth. Union Elcetcic elected to plug two (2) additional tubes with 38% indications.
(b) Steam Generator 'D*:
One hundred percent of the unplugged tubes or five thousand six hundred fourteen (5614) tubes were tested full length using the bobbin coil technique. The technique utilized frequencies of 630, 300, 150, and 30 kilohertz. In Steam Generator
'D', 402 tubes were inspected with a 0.560 KRPC probe at various intersections. No discontinuities were detected.
Sixteen (16) tubes were plugged in Steam Generator
'D'.
Thirteen (13) tubes were plugged when anti vibration bar indications exceeded 40 percent. Three (3) tubes were plugged with indications of 38 and 39 percent.
Additional testing was performed on these tubes using a (8X1) probe to determine if the tube wall loss at the AVB was one sided or two sided. The results of the testing showed only one sided wear on all sixteen (16) tubes.
