50-328 SEQUOYAH NUCLEAR PLANT (SQN) - NRC B'JLLETIN 89-01, SUPPLEMENT 2 - FAILURE OF WESTINGHOUSE ELECTRIC CORP 0 RATION STEAM CENERATOR (SG) TUBE MECRANICAL PLUGS This letter providos SQN's response to NRC Bulletin 89-01, Supplement 2, regarding the f ailure of Westinghouse SG tube mechanical plugs fabricated from the thermally treated Inconel 600 (1-600).
As identifled in Enclosures 1 through 3 TVA has been systematically removing all of Westinghouse I-600 SG tube plugs from service and replacing them with Inconel 690 tube plugs. To date, all 1-600 plugs have been removed from Unit 2, whereas 79 of 126 have been removed from Unit 1.
Of the remaining 47 I-600 plugs in Unit 1, only 10 are installed in hot legs.
These 10 are irom heat NX-5222 and are installed in SG No. 1.
A justification for continued operation fcr thcae 10 hot leg plugs was 1
previously submitted on September 17, 1990. The balance of the plugs

(37) is in the cold legs and does not require corrective actions until the year 2009 or later..1n keeping with TVA's conservative and proactive philosophy, however, we are making every effort to remove all of the I-600 plugs at the earliest possible time.

An " Application for Withholding Propeletary Information from Public Disclosuro" and an accompanying Af fidavit ( AW-91-167) were submitted by Woctinghouse in a May 31, 1991, letter to the NRC, (Attention: J. E. Richardson, Director, Division of Engineering & System Technology) in accordance with 10 CFR Section 2.790.
This request was made regarding WCAP-12244, Revision 3 Addendum 2, from which the noted enclosure was taken.
Therefore, it is requested that the noted enclosure be handled in accordance with the May 31, 1991, application for withholding and the accompanying affidavit.
