Commitments on Very High Radiation Area (VHRA) Access Controls This letter is written to provide status of commitments related to VHRA personnel access controls made by Omaha Public Power District (OPPD).

Inspection Report 78-07 Commitments made by 0 PPD in the response (Reference 3 included the following:

For Room 62, a local alarm will be installed on the existing door (hatch) to provide additional access control.
This will be completed as soon as design, procurement, and installation are accomplished, hopefully by the time the security system is operational.
2.
A locked gate will be installed across the hallway leading to the Personnel Air Lock (PAL) door. This gate will be alarmed at both the Central Alarm Station in the Guard Control Center and at the Secondary Alarm Station in the Control Room. The present lock on the PAL Door will continue to be utilized.
The following updated information is provided regarding VHRA access control commitments.
Room 62 Hatch Local Alarm In early 1990, as a result of a verification completed on Safety Enhancement Progran (SEP) Item No. 55, personnel investigated the alarm configuration on the Rcom 62 hatch and identified the commitments made in Reference 3.
This investigation revealed that the local alarm on the hatch to Room 62 had not been installed during the original security system installation as committed. An Incident Report was subsequently written regarding the Room 69 hatch alarm.

U. S. Nuclear Regulatory Commission LIC-91-160R Page 2 Although no specific cause could be pinpointed, deficiencies were found in the areas of open commitment research, commitment tracking, the use of maintenance work orders versus modification requests, and communications.
Since 1978, the systems, processes and performance in all of these areas have improved significantly at OPPD, The local alarm for the Room 62 hatch was subsequently installed in June 1990 under Field Design Change Request #90-834 to modification MR-FC-85-49, " Site Security Upgrade " Monthly Preventive Maintenance Procedure GM-PM-MX-0501 is utilized to periodically test the operability of this alarm.
Personnel Air Lock Door The commitment to install a locked gate and remote alarm across the hallway leading to the PAL door was implemented under design change FC-77-045 during 1978.
Since that time, the alarmed gate has been removed due to the interference with the movement of equipment into the containment building.
The magnetic alarm switch was relocated onto the PAL door itself under design change FC-84-010 in 1984.
Modification MR-FC-85-49 upgraded the alarm hardware to be compatible with the new security system. This alarm is annunciated at a remote location as described in Reference 3, but the present alarm hardware is directly mounted to the PAL door, not to a separate ctge door.
In addition to the alarm, the PAL door is maintained padlocked wheneve the containment building is considered a VHRA, except during entries at power.
At these times, a security officer is posted to provide area access control.
The combination of the PAL door remote alarm, padlocks, and limit 2d posting of security officers is presently considered by 0 PPD adequate for preventing an individual from inadvertently entering the containment building when it is posted as a VHRA.
All VHRA Access Control Commitments Other specific commitments for VHRA personnel access control have been made by 0 PPD since 1978 (e g., Reference 4 response to violations cited in Inspection Report 88-05, and SEP Item 55 correspondence).
Over the years, OPPD has implemented a combination of physical plant modifications and administrative controls which assure 3 roper control of personnel access to VHRAs; many of the specific elements of t11s VHRA personnel access control program reflect the commitments noted previously.
This control program may evolve further in the future as needed to implement company and regulatory goals.
Because of recognized improvements in this area and the desire to preclude repeated commitment update correspondence, such as Reference 5, OPPD elects to supersede specific previous commitments for the VHRA personnel access control program with the on-going commitment of continued implementation of an effective control program.
Although the basic control methods will still be used, this new commitment will promote continued improvements through refinement of specific program elements.
Any specific changes implemented will not reduce the overall effectiveness of the program.
Routine periodic inspections by Region IV personnel will provide adequate assessment and understanding of the program elements by the NRC.
