This int erim repo rt is in reference to the significant de fici ency in let ter 2NRC-3-014, regarding our Quality Control program initially discussed dated March 18, 1983, Interim Report - Significant Deficiency 83-02.
Duque sne Light Company is cont inuing to evaluate the work pe rfo rmed by the Quality Control Inspector involved and other actions described in t he at t ach ed repo rt.
Pur suant to the requirements of 10CFR50.55(e), it is anticipated that a s ub s equent report in this matter should be submit ted to the Region I of fice by Novembe r 1, 1983.

7 s-o BEAVER VALLEY POWER STATION - UNIT NO. 2 DUQUESNE LIGHT COMPANY Interim Report No. 4 on Potential Deficiency Related to Quality Control Acceptance of Non-Conforming Conditions 1.
SUMMARY
During the pr epara t ion fo r cab le pulli ng, it was es t ab li shed that the Quality Control (QC) Status Report,
showing acceptance of Condui t 2CK606NAl and S uppo rt 221-V-2 by QC Ins pecto r "H",
was not cons is tent with the cable raceway ticket status.
The ins tallation was phys ic ally verified, and the referenced conduit and support had not been ins talled as requi red for QC acceptance.
Following this incide nt, it was then e s tab li shed that Conduit 2CX951BB had been accepted by QC Inspector "H"
under the following conditions:
a.
An outstanding Nonconformance and Disposition Report was issued fo r the support.
b.
A Construction Deficiency Report was outstanding for the conduit.
c.
The appropriate Reject Tags were attached to the conduit and support.
2.
IMMEDIATE ACTION TAKEN Duquesne Light Company not ified the NRC Region 1 Of fice that an evalua-t ion under 10CFR50.55(e) was underway on February 17, 1983.
Addi t ional details were provided to the Region I Of fice in three previous Interim Report s on QC Acceptance of Conditions Not Conforming to Inspection Plan At tribut es and/or Drawings.
This interim repo rt pr ov ides ad di t ional info rmat ion rega rd ing the res ult s of the continuing evaluation of the ins pect ions performed by the QC inspector involved.

DESCRIPTION OF DEFICIENCY Failure of QC supervision to adequately check on the performance of an individual inspector involved in the verification of conduit and associ-ated suppo rt ins t al la t ion.
This failure was caused by a gradual shif t in emphas is from QC field supe rvis ion to QC ins pe ct ion s tatu s and scheduling.
4.
CORRECTIVE ACTION TO REMEDY DEFICIENCIES The reins pect ion of ins tallation work ac cep t ed by the QC ins pect o r is cont inuing.
The results of the reins pect ion, since the las t repo rt, is as fo llows :

+e In addition to the above, 150 items which included work accepted by the QC inspector involved was also inspected during the normal course of completion inspection. The number of attributes or errors has not been

included, but the ins pect ion followed our ins truct io ns to reins pect at any time work performed by the referenced inspector.
The types of errars were similar to that de sc ribed in our Interim Report No. 1 on this subject.
It is cons ide red s ign i fica nt that no fur t he r omissions have been found, and from the lot select ed, the amount of e r rors 1.as decreased from the amount originally discovered and reported l
as a potential significant de fic iency.
The program for the reins pect ion of this ins pector's work is cont inuing.

238 1.5 Comparing the total with the recent 3-month period of reinspection, it does appear that the error ratio approximates 1.5%.

It is expect ed that the next repo rt will be issued to the Region 1 Of fice by November 1, 1983.
