ATTACHMENT I RESPONSE TO APPENDIX A Violation A j

10 CFR 50, Appendix B, Criterion V requires that activities affecting.

quality shall be prescribed by documented instructions, procedures or drawings, of a type appropriate to the circumstances.

Bechtel Job Rule 8031-JR-M-17 requires that mechancial shock arrestors " installed in the horizontal position shall be labeled " Keep Off" to prevent them from being used as a step or ladder during construction.

Those requiring additional 4

temporary wrappings during construction operations shall be labeled with visible " Keep Off" signs."

Contrary to the above, on June 6, 1983, it was identified that horizontal shock arrestors, GBB-102-H34 and HBB-118-H40, were not labeled with " Keep Off" signs as required by the applicable project procedures.

Response to Violation A i

The pipe shock arrestors GBB-102-H34 and HBB-118-H40 have been labeled as required. Also, the board was removed on HBB-188-76.

Each of these snubbers have been satisfacto'rily inspected and stroked, hence, no damage

resulted from the storage conditions.

t To prevent recurrence, the responsible superintendents, general foremen and foremen were.re-instructed in the requirements of Job Rule 8031-JR-M-17.
