Design and Cerdlicadon Program, visited the Nuclear Engineering Laboratory of Toshiba Corporation in Kawasaki, Japan, on the afternoon of March 14,1994. The purpose of this visit was to review the GINAFFE testa j conducted by Toshiba over the last four years for compliance with Toshiba's l

" Quality Assurance Program for Simplified Boiling Water Reactor". AS50092, i Rev.0, dated December 16,1993. S. Yokobori, W. Mirumschi, H. Tonegawa, T. i Kurita, T. Tobimatsu, K. Arai, and K. Watanabe, all of the Toshiba Corporation, were contacted during the vielt.

The results of this review and the ongite witness and reports of the performance of the GIRAFFE tests by Karen Vierow, a GE engineer, affirms that the GIRAFFE tests were conducted in a high quality, disciplined, .<

Tobimatsu, K. Arai, and K. Watanabe of Toshiba at Toshiba's Nuclear Enfeering Laboratory in Kawasaki, Japan, on the afternoon of 5/14/94 to renew the QA pmgrun applied to th GIRAFFE testa conducted over the last four years. The GIRAFFE testin[ p'r was reviewed for compliance with the applicable portions of Toshisas ality Assurance Program for Simplified Boiling Water Reactor", AS. 2, Rev.0, dated December 16,1993. ,

Comments relevant to specific sections of the program are noted below. I Secdon 2.4.1 Self audiu were held annually in November over at least the last 4 years to evaluate the quality of work produced by the Nuclear Engineering Laboratory.

These audits were led by the laboratory's General Manager, Dr. Miyazawa, and i included senior staff from each major laboratory technology group.

Section 2.4.2 About 80 meetings were held over the lut 4 years between laboratory, engineering, and projects personnel to assure that the GIRAFFE test designs and test performance met engineering and project requirements. The last meedng was held 1/27/94 and had $ laboratory technology group,4 engineering department, and 1 project persons in attendance.

Section 3 Generally, documents and drawings are kept 10 years and then microfilmed for permanent storage.

Section 4 The initial specification of the test procedures for GIRAFFE were issued 2/10/89. MMor revisions were made and issued 5/26/89 and 7/4/89 to reflect engineering and project comments. Drawings were verified by design review and signed by middle and upper management.

Section 5 Equipment from outside vendors was procured according to procedure GG S01. It was most recently revised 12/1/93. QC inspected equipment arriving at the laboratory for packaging demage, visual. damage, and quantity of items.

Other specialised attributes were evaluated by the experimenters.

Toshiba independently veriEed the volumes of equipment and components supplied by vendors and used in the test facility by filling and draining measured amounts of water.

Section 6 Laboratory instruments were calibrated annually and stickers affixed to indicate the next date of calibration. Calibrations by Toshiba were traceable to national standards and calibration records were retained. Special electronic data reduction equipment used for GIRAFFE was calibrated periodically. This 2 .

calibration consisted of applying known and constant voltages to the equipment input and verifying the linearity of the output. -

he Bowmeter on the GIRAFFE program was supplied by a Toshiba subsidiary company. The manufacturer's certification of calibration was accepted without additional vedBcation at the laboratory. Thermocou sles were procured commercially with manufacturer's certifications anc, not tested.

I Pressure vessels were inspected and pressure tested annually. Test personnel  !

were certified by national test to perform high temperature and pressure  !

testing. GIRAFFE had two dedicated technicians trained and certiBed for this  !

high pressure and high temperature testing. -l Test plans were prepared and approved. Depending on the test objectives, the apprcwals included Toshiba, the.lointJapanese unl des, and/or GE.

Test procedures included a pretest checklist and an initial conditions check list. There was much evidence of planning tests and defining initial condidons and test configuration by group discussions and reviews. Many preop tesdng procedures and logs were shown as well as minutes ofjoint i GE/Toshiba meedugs to plan tests.

l Log sheets documented initial conditions and strip chart tecorders showed system changes with time. Test durations were from 8 to 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />. _ Data was electronically sampled every second since initial tests indicated that sigrdficant data changes occurred only at times greater than one minute.

Facility drawings and records of ae built conditicr., were presented and examined. The repentability ofimportant tenu was checked monthly during pedods of active testing.

tube.
