Dear Mr. Keppler:
This letter telecopied to your office, provides written confirmation of a prompt notification pursuant to DAEC Technical Specifications, Appendix A, Section 6.11.2.a.7.
On 9/4/83, the HPCI minimum flow return to torus line throttle valve was found to be opening intermittently. HPCI was declared inoperable at 1105 hours0.0128 days <br />0.307 hours <br />0.00183 weeks <br />4.204525e-4 months <br /> due to this problem and a seven day LC0 was entered. On 9/4, while conducting the daily operability test required by T.S. 4.5.D.2, it was determined that the RCIC outboard isolation valve would not close. The inboard valve was closed and RCIC declared inoperable. A 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> LC0 was declared in accordance with T.S. 3.5.D.3 and an orderly shutdown initiated.
An unusual event was declared and notifications were made to the NRC pursuant to Technical Specification 6.11.2.a and 10 CFR 50.72.
