57300,-JOB E-5590, FILE NO. 956C, E-44 This letter constitutes a final report concerning the subject condition initially reported to the Commission on May 24, 1983 as a reportable deficiency under the requirements of 10CFR50.55 (e).
On June 24, 1983, in a phone conversation between G. C. Ficke and E.
R.
Schweibinz, CG&E received an extension of the submittal date for this report to July 1, 1983.
The subject deficiency occurred due to I-T-E Imperial Corporation, manufacturer of the 4160 and 480 Volt load centers, deviating from the design specifications during manufacturing with regard to the wire size.
The load centers were manufactured at the I-T-E facility at Spring House, Pennsylvania.
The condition was initially reported on Nonconformance Report (NR) E-263, dated July 29, 1975, which was subsequently voided and superseded by NR E-312, Revision 1, dated October 28, 1975.
This NR was dispositioned for rework which was completed and accepted on March 5, 1976.
During a review of voided nonconformance reports by Task VII of the Quality Confirmation Program, the NRC resident inspector requested that NR E-312 be reviewed for reportability under 10CFR50.55 (e).
The identified deficiencies, results of our evaluation, and the corrective actions taken are as follows:

Fifteen substation compartments were identified where 4.16kV terminations were made with wire rated for 600 Volt service.

Keppler Regional Administrator June 30, 1983 LOZ-83-0062 Page 2 the cables for the three phase connection were bundled together and passed through a metal barrier protected by only a rubber grommet.
Had this not been corrected, the cables would have flashed to ground when energized.
The 600 Volt cable was replaced with 5kV cable and the vendor was notified.

Nine 480 Volt substations were identified where #14AWG wire was used rather than wire suitable for 100 ampere service.
The length of wire involved was short, so no voltage problem existed.
However, the #14AWG wire would have been subject to overheating, resulting in accelerated aging which could have led to a common mode failure of the substations.
The #14AWG wire was replaced with 1/0 flexible wire and the vendor notified.
Quality Confirmation Program Procedure 19-QA-18 has been revised to require that voided nonconformance reports be reviewed for repor tability under 10CFR50.55 (e).
Those voided nonconformance reports previously accepted by Task VII of the QCP will also be reviewed for 10CFR50.55 (e) reporting.
In addition, procedures and checklists governing the review of past Nonconformance Reports (NRs), Condition Evaluation Requests (CERs), Corrective Action Requests (CARS), Audit Finding Reports (AFRs), and Engineering Change Requests (ECRS) are being reviewed to ensure explicit instructions are contained therein so that document reviewers identify potentially reportable 10CFR50.55 (e) deficiencies.
