NRC COMBINED INSPECTION 50-272/83-01, 50-311/83-01 SALEM GENERATING STATION UNIT NOS. 1 AND 2 JANUARY 3 THRU 7, 1983 In response to your request for more accurate information concerning Item A in our violation response, dated November 19, 1982, we submit the following additional information:

Item of Violation Item A 10CFR50, Appendix B, Criterion XII, " Control of Measuring and Test Equipment" states, " Measures shall'be established to assure that tools, gauges, instruments, and other measuring and testing devices used in activities affecting quality are properly controlled, calibrated, and adjusted at specified periods to maintain accuracy within necessary limits."

Licensee Administrative Procedure (AP)-22, Measuring and Test Equipment Calibration Program, Revision 5, October 19, 1981 specifies that all measuring and test equipment shall be calibrated, shall be accounted for, shall be inventoried, and shall be properly stored:

Contrary to the above:

On August 27, 1982, multi-ampere tester M-137 was observed to have a calibration sticker with a calibration date of March 23, 1982.

However, the calibration data sheet for tester M-137 dated March 23, 1982, showed that the tester was found and was left in an out-of-calibration condition.

Nuclear Regulatory Commission 4/15/83 Reply to Item A In addition to the facts delineated in the Meeting Summary section of Inspection 50-272/83-01, 50-311/83-01, the following actions were taken in order to resolve the problem of the questionable calibration of meter M-137:

Immediately af ter the discovery of the inaccurate statement to the NRC concerning the calibration status of meter M-137, the meter was returned to the calibration vendor for an estimate of cost for the repair of the meter.

As the estimate was extremely high, the meter was recalled to Salem for shipment to the manufacturer for a similar estimate.

The meter was returned to Salem on February 23, 1983, and a reject tag was placed on the meter immediately.

On March 14, 1983 the meter was forwarded to the storeroom for shipment to the manufacturer.

It was shipped off-site on April 5, 1983.

The meter is still in the possession of the manufacturer being evaluated.

In order to assure future statements to the NRC are correct and accurate, the following actions have been instituted:

1. All Station supervisory personnel shall be officially notified of the importance of correct and accurate information in NRC violation responses and LERs, and the importance of their complete and cooperative participation in the preparations thereof.

Also, they are to be informed of the importance of proper documentation of information.

2.

Response request forms shall be modified to include signatures of the Supervisor and Sr. Supervisor of the responding department.

This will serve to guarantee accuracy of information.
