Dear Mr. Allan:
On March 30, 1983, in accordance with 10CFR.55 (e), we reported verbally to Region 1 a potential deficiency concerning two Motor Operated Globe Valves located in the Residual Heat Removal System, which failed to operate properly.
This letter serves as our 30-Day written report on this Potential Deficiency.
Description of Potential Deficiency The two globe valves which failed were manufactured by the Anchor Darling Valve Company of Williamsport, Pennsylvania and are iden-tified by Anchor Darling Company designations: "A/D E5836-20-1" and "A/D E5836-4-1", (Shoreham designations are lEll*MOV-042A and 1 Ell *MOV-038A).
These globe valves are located in Shoreham's Residual Heat Removal System.
The first indication of a problem occurred during testing of valve 1 Ell *MOV-038A when it was discovered that the stem clamp was missing.
Although this was considered an isolated case, the appropriate startup procedure was revised to include a check to insure that the stem clamp was properly oriented on the stem, with the key properly installed, and the setscrew properly tightened.
More recently, a similar problem was encountered when, during startup testing, it was discovered that valve lEll*MOV-042 had failed to stroke, although the open/ closed position indicator lights signalled that the valve had stroked.
This was because the position indicating lights on the control panels are actuated through limit switches which are associated with the motor operator.

4 SNRC-880 May 6, 1983 Page 2 caused by.?.he valve stem clamp setscrew loosening, allowing the stem clamp to slide along the stem and the stem clamp key to fall from its keyway.
This permitt'ed the stem to rotate with the stem nut, thus preventing the valve from stroking.
It is believed that the setscrew in the stem clamp loosened due to normal system vibration.
Upon being notified by Stone & Webster Engineering Corporation that other Anchor Darling valves at Shoreham may be subject to similar failures, it was determined that a Potential Reportable Condition under 10CFR50.55(e) exists.
Corrective Action
~
Stone ' & Webster has performed a revie'.' of Shoreham's safety-related valves and' has determined that, at Shoreham, only globe valves manufactured by Anchor Darling are susceptible to the type of failure described above.
Shoreham has 14 valves of this deeign.
Anchor Darling has provided a procedure, which may be used to preclude any future possibility of stem clamp setscrews becoming loose as a result of vibration.
The procedure involves tighten-ing the setscrew securely and then peening the thread _to provide a positive locking mechanism.
An E&DCR will be issued to insure that the stem clamp setscrei in' all of Shoreham's Anchor ~ Darling globe valves in Category 1 ser-vice are locked by the method described above.
This locking procedure will be incorporated into the apprcpriate reference manuals should dinassembly/ reassembly of any vaa.ve of this design be required.
The peening of the setscrews for all 14 valves will be completed prior to fuel load
