Dear Mr. Martin:

Your letter of May 13, 1983, forwarded Combined Inspection Report 50-277/93-10 and 50-279/93-10.

Apnendix A addresses one item which does not appear to be in full compliance with Nuclear Requiatory Commission requirements.

This item is restated below along with our response.

10 CFR 50, Appendix B, Criterion XII, requires that " Measures shall be established to assure that tools, pages, instruments, and other measuring and testing devices used in activities af fecting quality are properly controlled, calibrated, and adjusted at specified periods to maintain accuracy within necessary limits."

Contrary to the above, on March 25, 195 i the operations section and the test section of the station staff had not established adequate measures to assure that tools, gages, instruments, and other measuring and testing devices used during surveillance testing (a quality-af fecting activity) were properly controlled, calibrated, adjusted in that:

No station procedure had been established to administrative 1y control the measuring and testing equipment for which the operations and testing sections were responsible.

Traceability of measuring and testing eauipment usage was not consistently maintained due to the absence signout prior to use and to the frequent lack of documentation of the eouipment used in the surveillance test procedures.

Specific examples of undocumented test eculpment use include the following:

-~ The stopwatch used during Surveillance Test (ST) 6.7 performed January 30, 1993, on Unit 2.

-- The voltmeter used during ST 8.2 performed March 2, 1993, on Unit 2.

-- The pressure gage used during ST 6.17.1 performed July 26, 1981, on Unit 2.

Stopwatches 53-8127 and 5?-9240 were stored together and available for use; however, stopwatch 53-3240 was within its calibration due date (May 24, 1983), while stopwatch 53-9217 had exceeded its calibration due date (December 24, 1991).

This is a Severity Level V violation.

RESPONSE

1 Measures to provide assurance of current calibration and traceability of instruments used during operations and test section survelliance testing have been established.

A letter has been sent to onerations personne.' which instructs them to record the instruments serial number and calibration expiration date on surveillance tests which require une of temporarv test instruments.

This interim measure will be used j

until these recording requirements are incorporated into the surveillance tests by March 1, 1994.

The test section has established adeauate traceability by requiring equipment signout prior to use and the recording of the test for which the equipment is used.
