COMANCHE PEAK STEAM ELECTRIC STATION (CPSES), UNIT 2 DOCKET NO. 50-446 REJECTABLE INDICATIONS IN CONTAINMENT ELECTRICAL PENETRATION STRUCTURAL WELOS SDAR CP-91-04 (INTERIM REPORT)
Gentlemen:
On June 24, 1991, TV Electric notified Mr. T. Reis of a deficiency involving rejectable indications in containment electrical penetration structural welds that were installed prior to the resumption of Unit 2 construction in January, 1991.
This is an interim report of a potentially reportable item under the provisior.s of 10CFR50.55(e).
As a part of the design validation process, a review of documentation for butt welds between the containment penetration sleeve and the weld neck flange for the electrical penetration assemblies identified the lack of volumetric examination in violation of ASME Code requirements. This condition was identified on Deficiency Report DR-C89-1851. Ultrasonic examination as required per the engineering disposition of the above Deficiency Report was performed on the total population of seventy-five (75) Unit 2 electrical penetrations.
Unit 1 electrical penetrations were evaluated prior to licensing and no rejectable indications were identified.
Due to the crown existing on the weld, the angle beam search unit was originally placed on the base metal with the beam directed tangentially into the weld.
These examinations resulted in the identification of rejectable indications on nineteen (19) electrical penetrations which required additional evaluations.
Subsequently, the wolds of the nineteen (19) electrical penetrations were ground flush with the base metal and reexamined with the beam directed axially into the weld.
The subsequent evaluations of the original nineteen (19) electrical penetrations identified only three (3) electr' cal penetrations with rejectable indications.
