This is to notify the US Nuclear Regulatory Cqmmlsslon that, in accordance with the provisions of 1 OCFR Part 21, we have Identified a potential Issue and are submitting this Interim report of the event.

Description:==

On January 14, 2020, Flowserve was Informed that a Model 38878-8, 3-way, 2-posltlon solenoid supplied to Seabrook Station for use on the feed water Isolation valve actuator failed bench test at the customer site on January 13, 2020. A second solenold valve, Modal 38878-1, failed bench test at the site on January 14, 2020. The failed solenoid valves were returned to _

Flowserve for evaluation. As received solenoid valves were tested at Flowserve and failed test Evaluation:

Flowserve Is currently Investigating the root cause(s) of the solenoid valve failures but will not be able to complete this investigation within the 60-day regulatory tlmeline due to dlfflcultles determining the root cause(s} of the failures. This interim report is being submitted per the requirements of 10 CFR 21 to notify the NRC of this possible deviation and to document Flawserve's continuing evaluation to determine If these falluros represent a substantial safety hazard. The evaluation of these solenoid valve failures will be complete by June 11, 2020.

Extent of Condition:

WIii be determined once root cause{~) of the solenoid velve failures ere known.

Corrective Actions:

Will be determined once root cause(s) of the solenoid valve failures are known.

Summation:

In summary, one Model 38878-8 and one Model 38678~1, 3~Way, 2-position solenoid valves failed bench test at Seabrook Station prior to assembly on the FWIV Actuator. The solenoid valves were returned to Flowserve and failed as received tests. Investigation In to the root cause{s) of the failures is ongoing. Evaluation of whether these failures represent deviations or failures to comply that would be associated with substantial safety hazards wlll be complete by June 11, 2020.
