Mr. James P. O'Reilly Special Report Page Two August 3, 1983 CAUSE OF THE MALFUNCTION On June 28, 1983, with the Plant in Mode 1, the Reactor Vessel Lower Channel of the Digital Metal Impact Monitoring System (DMIMS) was declared inoperable when Sensor 751 failed a Channel Check.
Redundant Sensor 750 could not be selected at this time to return the channel to operable status since it had previously failed on June 16, 1983 DMIMS is required to be operable while in Modes 1 and 2 and this report is being submitted in accordance with Action Statement (a) of Technical Specification 3 3.3 10.
The exact cause of the channel failure is unknown at this time.
Maintenance personnel investigating the problem determined that the operation of the Microprocessor portion of the DMIMS was normal and that the failure was in the field equipment which contains accelerometers, cabling, and the pre-amplifier assembly.
PLANS AND SCHEDULE FOR RESTORING TO OPERABLE STATUS The field equipment for the Reactor Vessel Lower Channel of the DMIMS will be repaired and returned to operable status during the next Cold Shutdown.
