This interim report is in reference to the potential significant defi-ciency regarding our Quality Control program discussed in letter 2NRC-3-014, dated March 18, 1983, Interim Report - Significant Deficiency 83-02.

Duquesne Light is continuing to evaluate the work performed by the Quality Control Inspector involved and other actions as described in the attached report.

Pursuant to the requirements of 10CFR50.55(e), it is anticipated that a subsequent report in this matter should be submitted to the Region I of fice by August 1, 1983.

SUMMARY

During the preparation for cable pulling, it was es tab lished that the Quality Control (QC) Status Report, showing acceptance of Conduit 2CK606NA1 and Support 221-V-2 by QC Inspector "H",

was not consistent with the cable raceway ticket status.

The installation was physically verified, and the referenced conduit and support had not been installed as required for QC acceptance.

Following this incident, it was then established that Conduit 2CX951BB had been accepted by QC Inspector "H"

under the following conditions:

was issued for a.

An outstanding Nonconformance and Disposition Report the support b.

A Construction Deficiency Report was outstanding for the conduit c.

The appropriate Reject Tags were attached to the conduit and support 2.

IMMEDIATE ACTION TAKEN Duquesne Light Company. notified the NRC Region I Of fice that an evalua-tion under 10CFR50.55(e) was underway on February 17, 1983.

Additional details were provided to the Region 1 Of fice in Interim Report No. 1 on QC Acceptance of Conditions Not Conforming to Inspection Plan Attributes and/or Drawings.

This interim report provides additional information regarding the results of the continuing evaluation of the inspections performed by the QC inspector involved and the progress with reference to our previously committed corrective action.

DESCRIPTION OF DEFICIENCY Failure of QC supervision to adequately check on the performance of an individual inspector involved in the verification of conduit and associ-i ated support installation.

This failure was caused by a gradual shift in emphasis from QC field supervision to QC inspection status and scheduling.

4.

ANALYSIS OF SAFETY IMPLICATION An engineering evaluation of the deficiencies found to date is in process.

The completion of an evaluation to establish whether errors could adversely ef fect the safety of the operation of the plant will be as deficiencies are found during the reinspection a long range project of the work performed by the QC inspector involved.

CORRECTIVE ACTION TO REMEDY DEFICIENCIES The reinspection of installation work accepted by the QC inspector is continuing.

The results of the reinspection, since the last report, is as follows:

18 0.8 The types of errors were similar to that described in our Interim Report No. 1 on this subject.

It is considered significant that no further omissions have been found and from the lot selected, the amount of errors is decreasing from the amount originally discovered and reported as a potential significant deficiency. The program for the reinspection of this inspector's work is continuing.

The basic reorganization of the Quality Control Section dealing with electrical inspection was initiated February 18, 1983.

The assignments ins pect ion's for the performance of adminis trat ive functions, such as status, scheduling, and problem resolution have been assigned to a specific group.

The QC Supervisor and lead inspectors have been verbally instructed in the need for their concentration on the quality of the work of those inspectors assigned to them.

A preliminary formal instruction has been issued that requires the lead inspectors to sample and report the quality of the inspections performed by the inspectors assigned to them.

6.

ADDITIONAL REPORTS A further report will be issued advising the results of the continuing reinspection of the work of the QC inspector involved.

The results of the engineering evaluation of the significance of errors and omissions will also be reported.

It is expected that the next report will be issued to the Region I Office by August 1, 1983.
