Prompt Notification of a Reportable Occurrence File: A-ll8a

Dear Mr. Keppler:

This letter, telecopied to your office, is intended to satisfy the re-quirement for prompt notification of a Reportable Occurrence in accordance with Specification 6.ll.2.a of the Duane Arnold Energy Center Technical Specifi-cations.

Technical Specification paragraph (s) violated:

3.1.A Description of Occurrence:

While in cold shutdown, with all rods inserted, the reactor mode switch was moved from the " Refuel" position to the " Shutdown" position. This should have caused a full scram. However, the "A" logic did not trip. A manual scram was inserted and a full scram was achieved.

Preliminary cause was determined to be failure of the "A" system shutdown scram reset interlock due to faulty solder connections.

This failure caused the Mode switch shutdown contacts to be bypassed.

Investigation into the root cause is continuing. The reactor is being maintained in cold shutdown.
